An Investigation of Correlation Between Diameter and Pressure of Pulmonary Artery in Pulmonary Hipertension Cases Due To Biomass Smoke

Abstract
Amaç: Biomass dumanı maruziyetine bağlı pulmoner arteriyel hipertansiyon (PAH) bölgemizde yaygındır. Bu olgularda bilgisayarlı toraks tomografisi (BTT)' nde ölçülen ana pulmoner arter çapı (APAÇ) ile ekokardiografide ölçülen pulmoner arter basıncı (PAB) arasındaki ilişkiyi araştırdıkGereç ve Yöntem:Biomass maruziyeti olan 109 bayan olgu ile çalışıldı. Olgular tanılarına göre sınıflandırıldı (KOAH, emboli, idiopatik PAH benzeri grup (İPAHBG) ve asemptomatik). Normal PAB'na sahip 10 olguluk kontrol grubu oluşturuldu. Olguların yaş, biomass maruziyet süresi (yıl) ve biomass maruziyet yoğunluğu (saat/yıl), ekokardiyografik tahmini sistolik PAB, toraks BT APAÇ ve hastalık tanıları kaydedildi. PAB ile APAÇ arasındaki ilişki Pearson korelasyon testi ile incelendi. ve BT ölçüleri çalışma grubu ile karşılaştırıldı. Bulgular: APAÇ kontrol ve çalışma grubunda 26.9 ± 5.1 mm ve 37.1 ± 6.4 mm ölçüldü, (p<0.001). PAB kontrol ve çalışma grubunda 22.7 ± 2.4 mm ve 57,3 ± 22 mm ölçüldü, (p<0.001). PAB ile APAÇ arasında anlamlı pozitif korelasyon saptandı (r = 0.634 p< 0.01).Sonuç:Sonuç olarak çalışmamız biomass maruziyeti olan olgularda BTT' de ölçülen ana pulmoner arter çapı genişliği 29 mm ve üzerinde olan olgularda pulmoner hipertansiyon düşünülmesi gerektiğini göstermiştir.

We analysed the relationship between the diameter of main pulmonary artery (DPA) measured with thorax computerized tomography (CT) and pulmonary arteriel pressure (PAP) measured with echocardiography in patients exposed to biomass.Method:The Study included four biomass smoke exposed groups composed of 99 female patients diagnosed as COPD, Pulmonary Embolism, Idiopathic Pulmonary Arterial Hypertension Like Group (IPAGLG), and Asymptomatics and a group of control composed of 10 female cases having normal PAP. The age of the patient, duration of exposure and its intensty (hour/year), PAP, DPA, and diagnoses were recorded. The correlation between PAP and DPA was examined with Pearson?s correlation test.Results:DPA in the control and the exposed groups was 26.9 ± 5.1 mm and 37.1 ± 6.4 mm respectively, (p<0.000).
