Abstract
Dış kaynak kullanımı hem özel sektörde hem de kamu sektöründe yaygın bir şekilde kullanılmaktadır ve genellikle işletmeler tarafından asıl işleri dışındaki işler için tercih edilmektedir. Bir kamu sektörü olarak sağlık hizmetleri de hem destek hem de tıbbi hizmet alanlarında dış kaynak kullanımından yararlanmaktadır. Bu çalışmanın temel amacı, sağlık sektöründe dış kaynak kullanımının nedenlerini, kalite ve verimliliğe etkilerini ve risklerini ortaya koymaktır. Araştırma ile Van İl Sağlık Müdürlüğü'ne bağlı kamu hastanelerinde görev yapan sözleşmeli ve bakanlık onaylı hastane yöneticilerinin dış hizmet kullanımına ilişkin algıları ve dış hizmet kullanımının hastane verimliliğine etkisi ölçülmeye çalışılmıştır. Bu araştırma, betimsel-analitik yöntemle yürütülmüş ve anket yöntemiyle gerçekleştirilmiştir. Araştırma evrenini 57 hastane yöneticisi oluşturmaktadır ve bu yöneticilerin 41'i bu çalışmaya katılmıştır. Verilerin analizinde SPSS programı kullanılmıştır. Araştırma sorularının test edilmesinde tanımlayıcı ve çıkarımsal istatistiksel yöntemler kullanılmıştır. Van'daki hastanelerde destek ve sağlık hizmetleri alanında dış kaynak kullanımının yüksek oranda tercih edildiği, ancak destek hizmetlerine göre tıbbi hizmetler alanında daha az dış kaynak kullanıldığı belirlenmiştir. Literatürde yer alan dış kaynak kullanım gerekçeleri ve riskleri ile araştırmadan elde edilen bulgular arasında yüksek oranda uyum olduğu ortaya çıkmıştır. Hastane yöneticilerinin değerlendirmelerine göre dış kaynak kullanımı ile sağlık hizmetlerinin kalitesi-verimliliği arasında pozitif yönde güçlü bir ilişki bulunmuştur. Dış kaynak tedarikçilerinden yöneticiler büyük bir memnuniyet duyarken, tedarikçilerle ilgili az sıklıkta problem yaşadıkları tespit edilmiştir. Sonuç olarak, dış kaynak kullanımının riskli yönleri olmasına rağmen kaliteyi ve verimliliği artırdığı anlaşılmaktadır. Sonuç olarak bu araştırma ile dış kaynak kullanımının riskli yönlerine rağmen kalite ve verimliliği artırdığı ortaya çıkmıştır. Bu nedenle, sağlık departmanları başta olmak üzere, dış kaynak kullanan sektörler riskleri azaltıcı önlemler almak zorundadır.
Outsourcing is widely used in both the private and public sectors and it is generally preferred by businesses for their non-core jobs. Health care as a public sector also benefits from outsourcing in both support and medical services areas. The main purpose of this study is to reveal the reasons for outsourcing, its effects on quality and productivity, and its risks in the healthcare sector. Through the research, it was tried to measure the perceptions of the hospital managers, working as a contracted and ministry-approved in hospitals affiliated to Van Provincial Health Directorate, about outsourcing and the effect of outsourcing on hospital productivity. This research was carried out with the descriptive-analytical method and was carried out with the survey method. The research population consists of 57 hospital manager and 41 of them participated in this study. SPSS program was used in the analysis of the data. Descriptive and inferential statistical methods were used in testing the research questions. It has been determined that outsourcing services are preferred in the field of support and health services in the hospitals in Van, but less outsourcing is used in the field of health services compared to support services. It has been determined that outsourcing is preferred at a high rate in the field of support and medical services in hospitals in Van, but less outsourcing is used in the field of medical services compared to support services. It was found out that there was a high agreement between the reasons and risks for outsourcing in the literature and the findings obtained from the research. According to the evaluations of the hospital's managers, a strong positive relationship was found between the use of outsourcing and the quality-efficiency of health services. While the managers of the outsourcing suppliers are highly satisfied, it has been determined that they rarely have problems with the suppliers. As a result, it has been revealed that outsourcing increases quality and productivity despite its risky aspects via this research. For this reason, sectors that use outsourcing, especially healthcare departments, have to take measures to reduce risks.
