Artroskopik Rotator Manşet Yırtığı Cerrahi Tedavisine Biceps Tenotomisinin Etkileri

Abstract

Giriş: Biceps tenotomisinin rotator manşet yırtığı tedavisine etkisi tartışma konusudur. Tenotominin; omuzda ve dirsekte kas kuvveti kaybına neden olmadığını bildiren; kolda yanma ve kozmetik kötü görünüme neden olduğunu bildiren yayınlar mevcut olup, propriosepsiyon üzerine etkisi günümüzde halen tartışılmaktadır. Biseps tenotomisi cerrahi teknik olarak kolay ve ucuz bir yöntemdir. Biz de çalışamızda bu ucuz ve kolay yöntemin rotator manşet onarımı üzerine etkisini araştırmayı hedefledik. Materyal Method: Van Yüzüncü Yıl Üniversitesi Dursun Odabaş Tıp Merkezinde 2018 Mayıs ile 2021 Aralık ayları arasında artroskopik olarak rotator manşet onarımı ve biseps tenotomisi yapılan ve en az bir yıllık klinik takibi olan hastaları Ortopedi ve Travmatoloji Polikliniğinde değerlendirildi. Çalışmaya katılan 51 hastanın omuz eklem hareket açıklıkları, omuz ve dirsek kas kuvvetleri, omuz proprioseptif duyuları, aktif şikayetleri, Popeyes (Temel Reis) bulgularının olup olmadığını değerlendirildi. Bulgular: Çalışmaya katılan hastların sağlam ve ameliyat edilmiş omuzları karşılaştırıldığında omuz eklem hareket açıkları arasında, omuz ve dirsek kas kuvvetleri arasında, omuz proprioseptif duyuları arasında istatistiksel olarak anlamlı fark saptanmadı. Hastaların ameliyat öncesi ve ameliyat sonrası ASES ve Constans skorları arasında anlamlı istatiksel fark saptandı (p<0,01). Sadece 1 hastada Popeye (Temel Reis) bulgusu saptandı. 1 hastada tenotomiye bağlı kolda geçmeyen yanma şikayeti saptandı. Tartışma ve Sonuç: Biseps tenotomisi ve rotator manşet onarımı bir yıllık takibi sonucunda hastaların sağlam ve ameliyat edilen kolları karşılaştırıldığında istatistiksel olarak anlamlı kas kuvveti kaybı, eklem hareket kısıtlılığı, proprioseptif duyu kaybı saptanamamıştır. Kolda kramp ve Popeye (Temel Reis) bulgusu beklenildiğinden az görülmüştür. Dolayısıyla basit ve ucuz bir teknik olan biseps tenotomisi düşünüldüğünden daha masum bir cerrahi yöntem olabilir.Introduction: The effect of biceps tenotomy on the treatment of rotator cuff injury is a controversy.It is still being discussed whether tenatomy causes muscle strength loss in the shoulder and elbow,or causes burning in the arm and cosmetically bad appearence of the arm and whether it affects proprioception of the shoulder. Biceps tenatomy is an easy and inexpensive surgical technique. In our study, we aimed to investigate the effect of this inexpensive and easy method on rotator cuff repair. Material Method: We evaluated patients who underwent arthroscopic rotator cuff repair and biceps tenotomy and had at least one year of clinical follow-up in the Orthopedics and Traumatology Polyclinic at Van Yüzüncü Yıl University Dursun Odabaş Medical Center between May 2018 and December 2021.We evaluated 51 patient who participated in our study to asses their shoulder joint range of motion,shoulder and elbow muscle strenght,proprioceptive senses of shoulder,active complaints and to see whether they had Popeye's findings. Results: When the intact and operated shoulder of the patient who participated in our study was compared, no statistically significant difference was found between the range of motion of both shoulders, between the muscle strength of both shoulders and elbows, and between the proprioceptive senses of both shoulders. A statistically significant difference was found between the preoperative and postoperative ASES and Constant scores of the patients (p<0.01).Popeye's sign was detected in only 1 patient who had complaint of ongoing burning in the arm due to the tenotomy. Discussion and Conclusion: As a result, no statistically significant loss of muscle strength, limitation of joint motion, and loss of proprioceptive sensation were found between the healty and operated arms of the patients after one-year follow-up of biceps tenotomy and rotator cuff repair.Cramp in the forearm and Popeye's sign were noticed less than expected. Therefore, biceps tenotomy, which is a simple and inexpensive technique, may be more innocent surgical method than is thought.
