Clinical Significance of Changes Serum Tumor Necrosis Factor Alpha and Interleukin-6 Levels in Preeclamptic Women
Abstract
ÖZET Bursal E, Preeklampsili Gebelerde Serum TNF a ve IL-6 Düzeylerindeki Değişikliklerin Klinik Önemi, Y.Y.Ü. Sağlık Bilimleri Enstitüsü Tıp Fakültesi Biyokimya Anabilim Dalı, Yüksek Lisans Tezi, Van- 2003. Bu çalışmada, preeklampsili gebelerle normal gebeler arasında serumda bulunan sitokinlerden TNF-a ve IL-6'nın seviyelerinde oluşabilecek değişikliklerin gözlemlenmesi amaçlanmıştır. Bu amaçla preeklampsi tanısı konmuş 25 hastada sabah ve gece olmak üzere 2 defa ve normal sağlıklı 25 gebede sabah 1 defa kan alınarak serumları ayrılmıştır. Serum TNF -a ve serum IL-6 düzeyleri kemillümensans yöntemle tayin edilmiştir. Bu çalışmada, preeklampsili ve sağlıklı gebeler arasında TNF-a düzeyleri arasında anlamlı bir farklılığın olmadığı fakat, IL-6 seviyesinin preeklampsili gebelerde sağlıklı gebelere göre anlamlı olarak düşük olduğu tespit edilmiştir. Ayrıca, preeklampsi grubunda, günlük ritimden dolayı olabilecek değişimleri tespit edebilmek için yaptığımız sabah ve gece alınan serumlarda TNF-a ve IL-6 seviyelerinin birbirinden farklı olmadığı tespit edilmiştir. Yine, preeklampsili gebelerde sabah alınan kanlarda TNF-a ve IL-6 seviyeleri arasında anlamlı bir korelasyon gözlemlenmezken, gece alınan kanlarda TNF-a ve IL-6 seviyeleri arasında anlamlı pozitif bir korelasyonun olduğu tespit edilmiştir. Bu bulgulara göre, preeklampsi tanısı konmuş gebelerde serum TNF- a ve IL-6 düzeylerinin ölçümünün, preeklampsi etyo-patogenezini ortaya koymada faydalı olmayacağı görüşüne varıldı. Preeklampsi patogenezinde sitokinlerin önemini ortaya çıkarabilecek bir sonuca ulaşabilmek için, hafif ve şiddetli preeklampsi tanısı konmuş hasta gruplarında ve daha fazla sayıda katılımın sağlanmasıyla yeni çalışmaların yapılmasının gerekli olduğu kanisim taşımaktayız. In this study it is aimed to observe the changes that can occur in the serum TNF-a and IL-6 levels in preeclampsia and in healthy pregnant women. For this aim, blood sample was taken from 25 healthy pregnant women once in the morning and from 25 preeclamptic pregnant patients twice, once in the morning and once at night and serum was separated. Levels of serum TNF-a and serum IL-6 were determined by Chemiluminescent method. In this study it was determined that there was no difference between TNF-a levels of healthy and preeclamptic pregnant women, IL-6 level of preeclamptic pregnant was significantly lower according to healthy ones, however we also found out that there was no difference between TNF-a and IL-6 levels of serum that had been taken from the preeclamptic group in the morning and at night to be able to follow the changes that could happen because of the daily rhythm. No correlation was observed between TNF-a and IL-6 levels of serum that had been taken in the morning. On the other hand it was found out that there was a significant positive correlation between TNF-a and IL-6 levels of serum that had been taken at night. According to finds, we conformed to be unusefull to measurement of levels serum TNFa and IL-6 determining etyopathogenesis of preeclampsia. We thinks that it is necessary to study with light and strong preeclamptic patient groups and to study with larger groups of patients to reach a result that will make the importance of cytokines in pathogenesis of preeclampsia.
