Abstract
Meme kanseri, kadınlardaki bütün kanserlerin yaklaşık olarak dörtte birini oluşturan, en çok görülen insan neoplazmlarından biridir. Meme kanserlerinden invaziv duktal karsinom, kadınlarda en sık görülen meme kanseridir. Bu tümörlerde erken tanı ve uygun tedavi yaşam süresini önemli ölçüde etkilemektedir. Bu nedenle son yıllardaki çalışmaların, meme kanserlerinde prognozu belirleme ve sistemik yayılımı önleme amacıyla hormonal ve kemoterapötik ajanlarla tedavide yoğunlaştığı görülmektedir. Sadece klasik parametrelerin prognozu ve tedaviyi yönlendirmede yeterli olmaması nedeniyle yeni, güvenilir belirleyicilere gerek vardır.Bcl-2, programlanmış hücre ölümünü regüle eden bir proteindir. Bu protein, meme kanserlerinin karsinogenetik proçesinde önemli bir mekanizma olarak görülmektedir ve meme kanserlerinin 2/3'ünde eksprese edilir. Bcl-2 ekspresyonunun; ER pozitifliği, düşük histolojik derece, mutant p53 yokluğu ve c-erbB-2 ekspresyonunun düşük düzeyleri ile birliktelik gösterdiği kabul edilmektedir.Katepsin D, dokularda yaygın varyasyonlarda tespit edilen lizozomal bir proteazdır. Ayrıca, katepsin D meme kanserinde aşırı eksprese edilebilen, östrojen tarafından regüle edilen bir proteindir.Çalışmamızda 50 invaziv duktal karsinom olgusu, bcl-2 ve katepsin D ile immünohistokimyasal olarak boyandı ve sonuçlar diğer prognostik faktörlerle karşılaştırılarak değerlendirildi.Sonuç olarak; progesteron reseptörü ve düşük nükleer skor (NS) ile bcl-2 ekspresyonu arasında güçlü pozitif bir ilişki tespit edilirken, katepsin D ve diğer prognostik faktörlerle, bcl-2 ekspresyonu arasında; güçlü pozitif bir ilişki tespit edilmedi. Bundan başka, perinöral invazyon, lenf damarı invazyonu, kan damarı invazyonu ve yüksek NS ile katepsin D ekspresyonu arasında aynı zamanda güçlü bir ilişki gözlenirken diğer prognostik faktörler, c-erbB2, progesteron reseptörü, östrojen reseptörü ile katepsin D ekspresyonu arasında güçlü bir ilişki gözlenmedi.Anahtar Kelimeler: İmmünohistokimya

Invasive ductal carcinoma than cancers of the breast is the most common cancer of the breast in women. In these tumors, early diagnosis and appropriate medical treatment is to influence markedly to life span. For this reason, cancers of the breast is occured intesify at therapy with chemotherapotic agents and hormonal treatment for the purpose of prevention systemic spread and prognosis determination of works in the last years. Just reliable determinatives are necessary, because of not to be adequate to orientate treatment and prognosis only classic parameters.Bcl-2 is a protein that regulate programmed cell death. This protein is showed important one mechanism at carcinogenetic process of cancers of the breast and, is expressed at two out of three of cancers of the breast. Expression of bcl-2 protein is acceptanced and correlated with low level of c-erbB-2 expression and mutant p53 absence and low histologic grade and oestrogen receptor positivity.Cathepsin D is a lysosomal protease that has been detected in a wide variety of tissue. In addition, cathepsin D is an estrogen-regulated protease that may be overexpressed in breast cancer.In the present study is stained bcl-2 and cathepsin D expression in 50 invasive ductal carcinomas by immunohistochemistrical, and evaluated the correlation with other prognostic factors.Consequently, a strong positive relationship was not detected between bcl-2 expression with other prognostic factors and cathepsin D protein, while a strong positive relationship was detected, between bcl-2 expression with low nuclear grade and progesterone receptor? status. Furthermore, a strong correlation was not observed between cathepsin D expression with estrogen receptor? status and progesterone receptor? status and c-erbB-2 oncogene amplification and other prognostic factors, while a strong correlation was also observed between cathepsin D expression with high nuclear grade and blood vessel invasion and lymph-vessel invasion and perineural invasion.Key Words: Immunohistochemistry
