Results of Open Reduction and Salter Innominate Osteotomy at Surgical Treatment of Developmental Hip Dysplasia

Abstract

Amaç: Gelişimsel kalça displazisi (GKD) tedavisinde yaygın olarak kullanılan açık redüksiyon ve Salter innominate osteotomisi tedavisi uyguladığımız hastaların klinik ve radyolojik sonuçlarını değerlendirdik.Hastalar ve yöntem: Çalışmada 2004?2012 yılları arasında GKD nedeniyle açık redüksiyon ve Salter innominate osteotomisi uyguladığımız hastalardan yeterli arşiv ve dosya bilgisine ulaşılan ve son kontrolleri yapılabilen 36 hasta değerlendirildi.Çalışmaya 36 hastanın toplam 45 kalçası dahil edildi. Hastaların 28'i (%77.77) kız ve 8'i (%22.33) erkekti. Ortalama yaş 28.08 (18-55 ) ay ve ortalama takip süresi 41.8 (6-105) ay idi. Ameliyat öncesi hastaların yakınlarından anamnez alınarak hastaların yaşları, aile hikâyesi, kundak kullanımı ve eşlik eden hastalıklar sorgulandı. Hastaların ameliyat öncesi, sonrası ve en son kontrollerindeki asetabular indeks açı ölçümleri yapılarak asetabular indeks düzelme yüzdesi hesaplandı. Hastaların son kontrollerinde, avasküler nekroz varlığı Kalamchi ve MacEven kriterlerine, klinik değerlendirme Modifiye McKay kriterlerine göre yapıldı.Çalışmamızda, ameliyat edilen kalçaların ameliyat öncesi Aİ'i ortalama 39.92 (35-47) derece olarak ölçüldü. Ameliyat sonrası Aİ değerleri ortalama 30.50 (20-35) dereceydi. En son kontrolde ölçülen ortalama Aİ 18.52 (10-30) derece olarak ölçüldü.. McKay klinik kriterlerine göre değerlendirildiğinde ise, 22 kalça (%49) mükemmel (Grup 1), 17 kalça (%38) iyi (Grup 2), 4 kalçada (%9) orta sonuç alındığı tespit edildi. İki kalçada ise hastalar serebral palsi nedeniyle mobilize olamadığından değerlendirilmedi. Kalamchi ve MacEwen femur bas?ı avasküler nekroz sınıflamasına göre 11 kalçada (%24.4) avasküler nekroz izlendi. Altı kalçada (%13.4) tip 1, 2(%4.4) kalçada tip 2, 2 kalçada (%4.4) tip3 ve 1 kalçada (%2.2) tip 4 avasküler nekroz izlendi. Diğer komplikasyonlardan ise 3 kalçada (%6.6) redislokasyon, 1 kalçada (%2.2) derin enfeksiyon ve 1 hastada (%2.2) suprakondiler femur kırığı gelişti.Sonuç: Çalışmamızdaki veriler ışığında 1.5 yas? üzerindeki GKD vakalarının cerrahi tedavisinde, açık redüksiyon ve Salter'ın innominate osteotomisinin birlikte uygulanması tercih edilmesi gereken etkin bir cerrahi tekniktir.

Aim: We evaluated clinical and radiological results from the patients who received the treatment of Open Reduction and Salter Innominate Osteotomy that are used as a common techniques in Development Displasia of the Hip.The Patients and Method: In this study, among the patients to whom we implemented Open Reduction and Salter Innominate Osteotomy due to developmental displasia o f the hip, we evaluated 36 patients between 2004-2012 about whom sufficient data archive and file could be gathered and whose last controls could be carried out.45 hips of 36 patients were studied. 28 (%77.77) of the patients were female and 8 (%22.33) of them were male. The mean age was 28.08 (18-55) months and the mean of follow-up time was 41.8 (6-105) months. Prior to operation, by obtaining anamnesis from relatives of the patients, we searched about the ages and whether there were any other accompanying illnessess and relatives who experienced developmental displasy of hip, whether the swaddle was used or not. Acetabular index (AI) angle was calculated in preoperative, postoperative and last controls and so percentage of acetabular recovery index was assessed. In the last controls of the patients, the presence of avascular necrosis was examined according to Kalamchi and MacEwen Criteria while Clinical assessment was executed according to Modified McKay Criteria .In this study, preoperative AI values of the hips that had been operated were 39.92 (35-47) on average. Postoperative AI values were 30.50 (20-35) on average. AI values obtained in the last controls were 18.52 (10-30) on average. As for evaluation according to McKay Clinical Criteria; 22 hips (%49) were perfect (group1), 17 hips (%38) were good (group 2) and 4 hips (%9) were satisfactory (group 3). 2 hips, however, couldn?t be evaluated since the patients were not mobilize due to cerebral palsy. According to Kalamchi and MacEwen Classification of Avascular Necrosis of femoral head, in 11 hips (%24.4) avascular necrosis was diagnosed. Of those hips, 6 ones (%13.4) had Type I, 2 ones (%4.4) had Type II, 2 ones (%4.4) had Type III and 1 hip (%2.2) had Type IV avascular necrosis. In other complications, in 3 hips (%6.6) redislocation and in 1 hip (%2.2) deep infection developed whereas 1(%2.2) patient had supracondilliar femur fracture.Result: As a result of this study, the co-applicaton of Open Reduction and Salter Innominate Osteotomy in surgical treatment of Developmental Displasy of Hip in cases over 1.5 year of age is a useful and preferrable surgical technique.
