Abstract
Giriş: Vitiligo, akkiz, idiyopatik, net sınırlı depigmente maküllerle karakterize bir hastalıktır. Klinikte lokalize, jeneralize, üniversal ve karma tipleri görülmektedir. Jeneralize vitiligoda fotokemoterapi en sık kullanılan yöntemdir. Son yıllarda yan etkisi az olan dar band UVB tedavisinin daha etkili olduğu bildirilmektedir.Amaç: Bu çalışmada dar band UVB, kalsipotriol-dar band UVB ve dar band UVB-kalsipotriol-betametazon kombinasyon tedavilerinin etkinliğini araştırdık.Materyal ve Metod: Çalışmaya Yüzüncü Yıl Üniversitesi Tıp Fakültesi Dermatoloji Anabilim Dalı Polikliniğine başvuran, nonsegemental vitiligo tanısı alan 45 hasta alındı. Hastalar 3 gruba ayrıldı. 1'inci grup betamatazon-kalsipotriol (Psorcutan beta® pomad)-dar band UVB ile tedavi edildi. 2'inci grup kalsipotriol (Psorcutan pomad®)-dar band UVB tedavisi 3. gruba ise sadece dar band UVB tedavisi uygulandı. Tedavi öncesinde her hastanın yaşı, cinsiyeti, mevcut lezyonların süresi, ailede vitiligo öyküsü hazırlanmış formlara kaydedildi. Hastalar haftada 3 seans şeklinde tedavi aldı. Vitiligo lezyonlarının fototipi bir olduğundan, hastaların deri fototipine bakılmaksızın tüm hastalarda başlangıç dozu 0.1 j/cm² şeklinde başlandı. Her seansta %10 oranında artırılarak 2.5 j/cm²'ye ulaşıldıktan sonra daha fazla arttırma yapılmadı. Hastaların tedavi öncesinde ve sonrasında psikososyal durumunu gösteren dermatoloji yaşam kalitesi formu ve görsel analog skalası formu düzenlendi.Bulgular: Hastaların yaşları 13 ile 55 (Ortalama 25.59) olarak saptandı. Hastalık süresi 3 ay ile 20 yıl arasında değişmekteydi. Tüm hastaların içinde aile öyküsü %22.2 oranında saptanmıştır (Ki-kare ( ? 2) = 0.257, p = 0.879 ). Tedaviden sonra düzelme oranları 1'inci grubun ortalaması %63.33±7.55, 2'inci grubun %60.67±5.75 ve 3 grubun ise %46.67±7.98 istatistiksel olarak anlamlı düzelme saptandı. Fakat gruplar arasında tedavi sonrası düzelme yüzdeleri açısından istatistik fark saptanmadı (p=0.221). Görsel analog skalası tedavi sonrasında 1'inci grupta ortalama değer 3.27±0.78, 2'inci grupta 3.20±0.56 ve 3'üncü grupta ise 4.07±0.78 olarak saptandı. Gruplar arasında görsel analog skalası tedavi sonrası değerleri açısından istatistiksel fark saptanmadı (p<0.01). Fakat grupların kendi içinde görsel analog skalası ortalama değerleri açısından istatistik olarak anlamlı düşüşler saptandı (p<0.01). Grupların kendi içinde Dermatoloji Yaşam Kalitesi İndeksi tedavi öncesi ve sonrası değerler açısında istatistik olarak anlamlı düşüşler saptandı. (p<0.01).Sonuç: Vitiligoda Dar band UVB ve kombine tedavileri etkili bulunmuştur. Tedavilerin dermatoloji yaşam kalitesi indeksinde belirgin düzelmeye neden olduğu görüldü. Background: Vitiligo is an idiopathic, acquired, well-demarcated disorder characterized by depigmented macules. There are clinical variants of localized, generalized, universal and mixed types. Photochemotherapy is the most commonly used treatment modality in generalized vitiligo. In recent years, narrow band UVB phototherapy that has fewer side effects is reported to be more effective.Objective: In this study, we investigated the effectiveness of narrow-band UVB, calcipotriol-narrow band UVB and narrow-band UVB-betamethasone combination therapies.Material and Methods: Forty five patients patients referred to dermatology policlinic of Yüzüncü Yıl University faculty of medicine were enrolled into this study. Patients were divided into 3 groups. First group was treated with betamethasone ?calcipotriol (Psorcutan beta® pomad)-narrow-band UVB phototherapy. Second group was treated with calcipotriol (Psorcutan pomad ®)-narrow-band UVB phototherapy and third group was treated only with narrow band UVB phototherapy. Before treatment age, gender, duration of existing lesions, family history of vitiligo were recorded on prepared forms for each patient. Patients were treated thrice weekly. Since phototype of vitiligo lesions was same, irrespective of patients phototype all patients received 0.1 j / cm ² as initial dose. Doses were increased by 10% in each session till we reached 2.5 j / cm ² after than no increase was made. Before and after treatment dermatology quality of life scale and visual analog scale assessments were performed to determine the psychological status.Results: Patients age range was 13-55 years (mean value 25.59). Duration of illness varied between 3 months to 20 years. Family history was at a rate of 22.2 % (Ki-square (c2) = 0.257, p = 0.879 ). Mean value recovery rates after treatment in the first group was 63.33±7.55 % in the second group was 60.67.±5.75 % and in third group was 46.67±7.98 % . There was statistically significant improvement. However, in terms of percentage of recovery there wasn?t statistical difference between the groups after treatment (p = 0,221). Post-treatment mean value visual analog scale values in the first group was 3.27±0.78, in the second group was 3.20±0.56 and in the third group was 4.07±0.78. There wasn?t statistical difference between groups for post-treatment visual analogue scale values (p <0.01). However, in each group itself the mean value visual analog scale values revealed statistically significant decrease (p <0.01). In each group itself pre- and post-treatment Dermatology Life Quality Index scores revealed statistically significant decrease (p <0.01).Conclusons: In vitiligo narrow band UVB phototherapy and combination treatments were found effective. It was observed that treatments cause significant improvement in dermatology life quality index.
