Investigation of Serum Cyclophilin a Levels in Patients With Gestational Diabetes
Abstract
Bu çalışmada, gestasyonel diabetes mellitus (GDM) tanısı almış ve öglisemik olan gebelerin klinik özelliklerinin karşılaştırılması ve serum siklofilin A düzeyinin GDM'yi öngörmede yerinin değerlendirilmesi amaçlanmıştır. Yöntem: Bu vaka kontrol çalışmasında Van Yüzüncü Yıl Üniversitesi Dursun Odabaşı Tıp Merkezi Kadın Doğum ve Hastalıkları Polikliniklerinde Nisan 2021-Mart 2022 tarihleri arasında tanı alan 90 GDM olgusu ve 90 öglisemik gebenin sonuçları değerlendirilmiştir. Olguların yaşı, beden kitle indeksi, obstetrik özellikleri ve siklofilin A dahil laboratuvar sonuçları kaydedilmiştir. Bulgular: Kontrol grubu ile karşılaştırıldığında, GDM olguları arasında üre, kan glikoz düzeyleri ve HbA1c değeri istatistiksel olarak anlamlı düzeyde daha fazlaydı (p<0,05). Kontrol grubunda ortalama siklofilin A düzeyi 23,88 ± 6,51 ng/mL, GDM grubunda ise 28,79 ± 6,17 ng/mL idi. Kontrol grubu ile karşılaştırıldığında GDM olguları arasında siklofilin A düzeyi istatistiksel olarak anlamlı düzeyde daha fazlaydı (p<0,001). Serum siklofilin A düzeyinin ideal olarak 24,14 ng/mL kesim noktasında GDM olgularını %84,3 sensitivite ve %72,6 spesifite ile saptayabildiği belirlendi (AUC: 0,813 [0,746 - 0,879], p<0,001). Siklofilin A düzeyi ile serum glikoz düzeyleri arasında düşük düzeyde pozitif yönde korelasyon ilişkisi gözlendi (p<0,05). Sonuç: Sonuç olarak, öglisemik gebelerle GDM'li gebeleri ayırt etmede serum siklofilin A düzeyinin 24,14 kesim noktasında kabul edilebilir bir prediktif değeri olduğu belirlenmiştir. Çalışmamızın bulguları ışığında, daha fazla gebenin dahil edildiği, çok merkezli, prospektif ve gebeliğin erken dönemlerinde siklofilin A düzeyinin kaydedildiği gelecek çalışmalarla serum siklofilin düzeyinin klinik pratikte GDM tanısı açısından kullanılabilirliği daha detaylı olarak ortaya konabilir.
In this study, it was aimed to compare the clinical features of pregnant women diagnosed with gestational diabetes mellitus (GDM) and pregnant women who are euglycemic and to evaluate the role of serum cyclophilin A level in predicting GDM. Method: In this case-control study, the results of 90 GDM cases and 90 euglycemic pregnant women who applied to Van Yüzüncü Yıl University Dursun Odabaşı Medical Center Obstetrics and Gynecology Polyclinics between the dates of April 2021 – March 2022. Age, body mass index, obstetric characteristics and laboratory results including cyclophilin A were recorded. Results: Compared with the control group urea, blood glucose levels and HbA1c values were statistically significantly higher among GDM cases (p< 0.05). The mean cyclophilin A level was 23.88 ± 6.51 ng/mL in the control group and 28.79 ± 6.17 ng/mL in the GDM group. Compared to the control group, cyclophilin A level was statistically significantly higher among GDM cases (p<0.001). It was determined that serum cyclophilin A level could ideally detect GDM cases with 84.3% sensitivity and 72.6% specificity at the cut-off point of 24.14 ng/mL (AUC: 0.813 [0.746 - 0.879], p<0.001). A low level of positive correlation was observed between cyclofin A and serum glucose values (p<0.05). Conclusion: In conclusion, it was determined that serum cyclophilin A level has an acceptable predictive value at the cut-off point of 24.14 in distinguishing euglycemic pregnants and pregnant women with GDM. In the light of the findings of our study, the usability of serum cyclophilin level in clinical practice for the diagnosis of GDM can be demonstrated in more detail with multicenter, prospective studies in which more pregnant women are included and cyclophilin A levels are recorded in the early period of pregnancy.
