Iron Deficiency Anemia Affect of Left Ventricular Diastolic Fonction
Abstract
Amaç: Hafif (Hb 7-10 g/dl) ve derin (Hb <7 g/dl) demir eksikliği anemisi olan bireylerde aneminin sol ventrikül diyastolik fonksiyon parametreleri üzerine olan etkisini değerlendirmek ve sağlıklı bireylerle karşılaştırmaktır.Materyal ve Metot: Ek hastalığı bulunmayan Hb değeri 7 g/dl'nin altında olan kronik derin demir eksikliği anemili 40 hasta ile Hb değeri 7-10 g/dl arasında olan kronik hafif demir eksikliği anemili 40 hasta alındı. Kontrol grubu olarak da çalışma grubuna benzer demografik özellikleri olan ve anemisi olmayan 20 sağlıklı gönüllü alındı. Hastaların tamamı başlangıçta anamnez, kardiyak risk faktörleri ve sistem sorgusu, kan basıncı, nabız ve sistemik fizik muayene ile değerlendirildi. Hastalarda hemogram ve kreatin bakıldı. EKG ve transtorasik EKO yapıldı. 12 derivasyonlu EKG ile kardiyak ritim değerlendirildi. EKO'da konvansiyonel tüm parametreler, mitral Akım PW Doppler, mitral kapak lateral anulustan PW Doku Doppler ve M-Mod Akım Yayılım Hızına bakıldı.Bulgular: Kontrol grubu ile karşılaştırıldığında anemi grubunda hemoglobin, hemotokrit, MCV anlamlı olarak daha düşüktü. Nabız daha yüksekti. Anemi grubunda sol atriyum çapı ve sol ventrikül end diastolik çapı anlamlı olarak daha büyüktü. Derin anemi grubunda 5 kişide, hafif anemi grubunda 7 kişide diastolik doluş bozukluğu saptandı.Sonuç: Anemi kalbin diastolik fonksiyonlarını olumsuz yönde etkilemektedir. Aneminin ciddiyetine parelel olarak kalbin sol ventrikül end diastolik çapı ve sol atriyum çapı artmakta, diastolik doluş parametreleri ise uzamaktadır. Bizim çalışmamızda benzer sonuçlar görüldü.
Aim: Investigation of the affect of anemia on diastolic echocardiographic parametrs in people having mild (Hb 7-10 g/dl) and deep anemia (Hb < 7 g/dl) and comparison these findings with non-anemic healthy people.Material and Method: 40 patients having iron- deficiency anemia whose Hb parameters are below 7 gr/dl without any other accompanying illness and 40 other light iron-deficiency anemic patients (Hb 7-10 g/dl ) are included in the study. As the control group 20 healthy volunteers having similar demographic charesteristics were recruited in the study. At the beginning all subjects were asssesed for medical history, cardiac risk factors and general systematic problems after which blood pressure and pulse were determined and a medical examination was performed. Hb and creatinine levels were determined in all patients. An ECG and transtorasic echocardiographic examination were performed on each subject. The cardiac rithym was determined by a 12-derivation ECG. In the echocardiographic examination, beside all conventional parameters, mitral valvular PW Doppler, mitral annulus PW tissue doppler and mitral valve M-mod propogation velocity examinations were done.Findings: In the anemic patients hemoglobin, hemotocrit and MCV were significantly compared to the control group. The heart rate was higher in the anemic patients. In the anemic patients the left atrial diameter and LV end diastolic diameter were significantly greater than the controls. Five patients in the deep and 7 patients in the mild anemic groups were found to have insufficient diastolic filling.Results: Anemia negatively affects the cardiac diastolic functions. According to the severity of anemia the left atrial diameter and LV end diastolic diameter increases and LV diastole prolongs. Our study has revealed comparable results with the literature.
