Abstract
41. ÖZETYüzüncü Yıl Üniversitesi Tıp Fakültesi Hastanesi Çocuk Sağlığı ve HastalıklarıAnabilim Dalı servisinde Eylül 2003 ile Temmuz 2005 tarihleri arasında klinik veradyolojik olarak pnömoni tanısı alan çocuklarda pulmoner hipertansiyon tedavisineyönelik çalışmamızda toplam 40 vakadan oluşan hasta grubu alındı. Pnömoni ile birliktekonjenital kalp hastalığı, sistemik hipertansiyon, böbrek veya karaciğer hastalığı olanvakalar çalışmaya dahil edilmedi.Vakaların tümünde kan gazları ve pulse oksimetre ile oksijen satürasyonlarıölçüldü. Ayrıca akciğer grafisi çekilip, elektrokardiyografik ve ekokardiyografik incelemeyapıldı.Ekokardiyografik değerlendirme M mode, 2 boyutlu ekokardiyografi ve renkliDoppler, pulse ve continius wave (CW) ile yapıldı. Ekokardiografik incelemede pulmonerarter basıncı 35 mmHg'nın üzerinde olan vakalar PH olarak kabul edildi. Ekokardiyografikincelemede PH tesbit edilen vakalar iki gruba ayrıldı. Birinci gruba kaptopril 2mg/kg/günüç dozda, ikinci gruba nifedipin 0.5 mg/kg/gün 3 dozda başlandı.. PH normal sınırlaradönünceye kadar hastalara her gün ekokardiografik ile inceleme yapıldı. Tedavide,vakaların hepsine başlangıçta ampirik olarak ikili antibiyotik tedavisi başlanıp, takiptekültür sonuçlarına göre gerekli görülen vakalarda antibiyotik değişimi yapıldı. Akciğerenfeksiyonu ile birlikte kalp yetmezliği olan vakalarda digoksin uygulandı.Çalışmanın istatistiksel olarak değerlendirilmesinde SPSS (Statistical Package forSocial Sciences) programında Bağımsız İki Örnek t testi, Bağımlı İki Örnek t testi, Ki-karetesti kullanıldı.

SUMMARYThis study which was included 40 cases in patient group, who were diagnosed withpneumonia and pulmonary hypertension from the radiographic and clinical features wasperformed from September 2003 to July 2005 in Pediatric Departmant of YYU MedicalFaculty. Patients which had pneumonia and congenital heart disease or systemichypertension or renal and liver disease together were excluded from the study.Blood gas analysis and oxygen concentration which was measured with pulseoxymetre were performed in all patients. Besides lung graphy draw andelectrocardiographic and echocardiographic search was done.Echocardiographic evoluation was done by echocardiographic and the by M mode,two dimentional continuous and colored doppler. At echocardiographic examination,pulmonary hypertension is defined as above 35 mmHg of pulmonary artery pressure. Inechocardiographic examination, patients with pulmonary hypertension are divided into 2groups. Captopril (2 mg/kg/day in tree dose in a day) and nifedipine (0.5 mg/kg/day in treedose in a day) were given the first and the second group respectively. Echocardiographywas performed daily until achieving normal pulmonary artery pressure. In the beginning ofthe treatment the patient were treated by double antibiotics treatment and antibiotic changewas done for the needed cases at the follow up. Digoksin was administered to the caseswho has lung infection with heart failure.Mann-Witney U and Ki-Square tests were used for the statistical evoluation of thestudy.
