Relationship Between Kellgren-Lawrence Radiographic Scores of Knee Joint Compartments and Knee Range of Motion, Knee Pain, Disability in the Knee Osteoarthritis

Abstract

Amaç: Bu çalışmanın amacı semptomatik diz osteoartritli hastalarda diz eklemi kompartımanlarının Kellgren-Lawrence radyografik skorları ile diz eklem hareket açıklığı, diz ağrısı ve disabilite arasındaki ilişkiyi değerlendirmektir.Gereç ve Yöntemler: Fiziksel Tıp ve Rehabilitasyon polikliniğine diz ağrısı nedeniyle başvuran ve ACR (American College of Rheumatology) tanı kriterlerine göre bilateral diz osteoartriti tanısı konulan 100 hasta çalışmaya dahil edildi. Hastalar, diz eklemlerinin antero-posterior, lateral ve tanjansiyel direkt grafileri çekilerek radyografik olarak değerlendirildi. Diz eklemleri, medial ve lateral tibiofemoral ve patellofemoral kompartımanlara ayrılarak her kompartıman ve her bir diz eklemi için kompartımanların Kellgren-Lawrence radyografik skorunun toplamı hesaplandı. Kellgren-Lawrence skorları ile diz eklem hareket açıklıkları, diz ağrısı, disabilite arasındaki korelasyonlar istatistik hesaplamalarla belirlendi. Değerlendirmelerde VAS (Vizüel analog skala), WOMAC (Western Ontario ve McMaster Universities Index) ve Stanford Health Assessment Questionnaire (HAQ) ölçekleri kullanıldı.Bulgular: Semptomatik diz osteoartritli hastaların diz eklem kompartımanlarına ait Kellgren-Lawrence radyografik evrelemesi bulguları ile klinik değerlendirme parametreleri (VAS, WOMAC, HAQ, EHA) arasında anlamlı bir korelasyon bulundu (p<0,001).Sonuç: Polikliniğimize başvuran semptomatik diz osteoartritli hastaların diz eklem kompartımanlarına ait Kellgren-Lawrence radyografik evrelemesi bulguları ile klinik değerlendirme parametreleri arasında anlamlı bir ilişki vardır. Semptomatik diz osteoartritli hastaların değerlendirilmesinde klinik ve radyografik veriler birlikte ele alınmalıdır.

Objective: The aim of this study is to evaluate the relationship between knee range of motion, knee pain and disability with Kellgren-Lawrence radiographic scores of knee joint compartments in patients with symptomatic knee osteoarthritis.Material and Methods: 100 patients, who admitted to Physical Medicine and Rehabilitation outpatient clinic with knee pain and diagnosed with bilateral knee osteoarthritis according to the ACR (American College ofRheumatology) criteria, were included in the study. Patients were evaluated radiographically by direct X-rays of knee joints with anterior-posterior, lateral and tangential views. The knee joints were divided into medial and lateral tibiofemoral and patellofemoral compartments and for each knee joint, the sum of Kellgren-Lawrence radiographic scores of the compartments were calculated. Using Kellgren-Lawrence scores, the correlations between the range of motion of the knee, knee pain and disability were determined with statistical calculations. In assessment, VAS (visual analogue scale), WOMAC (Western Ontario and McMaster Universities Index) and the Stanford Health Assessment Questionnaire (HAQ) scores were used.Results: A significant correlation was determined between the results of Kellgren-Lawrence radiographic staging of the knee joint compartments and the clinical assessment parameters (VAS, WOMAC, HAQ, EHA) of patients with symptomatic knee osteoarthritis (p <0.001).Conclusion: There is a significant relationship between the results of Kellgren-Lawrence radiographic staging of the compartments of the knee joints and clinical evaluation parameters in patients that admitted to our outpatient clinic with symptomatic knee osteoarthritis. Clinical and radiographic data should be considered together to evaluate the patients with symptomatic osteoarthritis of the knee.
