Article Clinical, Demographic, and Radiological Characteristics of Patients Demonstrating Antibodies Against Myelin Oligodendrocyte Glycoprotein(Galenos Publ House, 2024) Koc, Sumeyye; Sen, Sedat; Terzi, Yuksel; Kizilay, Ferah; Demir, Serkan; Aksoy, Durdane Bekar; Terzi, MuratBackground: Optic neuritis, myelitis, and neuromyelitis optica spectrum disorder (NMOSD) have been associated with antibodies against myelin oligodendrocyte glycoprotein-immunoglobulin G (anti-MOG-IgG). Furthermore, patients with radiological and demographic features atypical for multiple sclerosis (MS) with optic neuritis and myelitis also demonstrate antibodies against aquaporin-4 and anti-MOG-IgG. However, data on the diagnosis, treatment, follow-up, and prognosis in patients with anti-MOG-IgG are limited. Aims: To evaluate the clinical, radiological, and demographic characteristics of patients with anti-MOG-IgG. Study Design: Multicenter, retrospective, observational study. Methods: Patients with blood samples demonstrating anti-MOG-IgG that had been evaluated at the Neuroimmunology laboratory at Ondokuz May & imath;s University's Faculty of Medicine were included in the study. Results: Of the 104 patients with anti-MOG-IgG, 56.7% were women and43.3% were men. Approximately 2.4% of the patients were diagnosed with MS, 15.8% with acute disseminated encephalomyelitis (ADEM), 39.4% with NMOSD, 31.3% with isolated optic neuritis, and 11.1% with isolated myelitis. Approximately 53.1% of patients with spinal involvement at clinical onset demonstrated a clinical course of NMOSD. Thereafter, 8.8% of these patients demonstrated a clinical course similar to MS and ADEM, and 28.1% demonstrated a clinical course of isolated myelitis. The response to acute attack treatment was lower and the disability was higher in patients aged > 40 years than patients aged < 40 years at clinical onset. Oligoclonal band was detected in 15.5% of the patients. Conclusion: For patients with NMOSD and without anti-NMO antibodies, the diagnosis is supported by the presence of anti-MOG-IgG. Furthermore, advanced age at clinical onset, Expanded Disability Status Scale (EDSS) score at clinical onset, spinal cord involvement, and number of attacks may be negative prognostic factors in patients with anti-MOG-IgG.Article Comparison of Microscopy, Elisa, and Real-Time Pcr for Detection of Giardia Intestinalis in Human Stool Specimens(Tubitak Scientific & Technological Research Council Turkey, 2017) Beyhan, Yunus Emre; Cengiz, Zeynep TasBackground/aim: This study included patients who had digestive system complaints between August 2015 and October 2015. The research was designed to compare conventional microscopy with an antigen detection ELISA kit and the TaqMan-based real-time PCR (RT-PCR) technique for detection of Giardia intestinalis in human stool specimens. Materials and methods: Samples were concentrated by formalin-ether sedimentation technique and microscopic examinations were carried out on wet mount slides. A commercially available ELISA kit (Giardia CELISA, Cellabs, Brookvale, Australia) was used for immunoassay. DNA was extracted from fecal samples of about 200 mg using the QIAamp Fast DNA Stool Mini Kit (QIAGEN, Hilden, Germany) and the LightCycler Nano system (Roche Diagnostics, Mannheim, Germany) was used for the TaqMan-based RT-PCR assay. Results: A total of 94 stool samples, 38 of them diagnosed positive (40.4%) and 56 of them diagnosed negative by microscopy, were selected for evaluation by antigen detection and molecular assays. The prevalence of G. intestinalis infection was found as 46.8% (n: 44) and 79.8% (n: 75) by ELISA and RT-PCR, respectively. RT-PCR revealed by far the highest positivity rate compared to the other two methods. The difference between these methods was found to be statistically significant (P < 0.05). In comparison to PCR, the sensitivity and specificity of microscopy and ELISA were 50.7% and 100% and 53.3% and 79%, respectively. Yöntem ve gereç: Bu çalışma çift kör, çok merkezli (GATA-Ankara ve 100. Yıl Üniversitesi-Van), plasebo kontrollü olarak yapıldı. Etik komite onayından sonra ASA I ve II sınıfı, yetişkin, 18-65 yaş arasında çalışmayı kabul eden 141 hasta çalışmaya alındı. İv. olarak hastalardan grup F’ye (n = 33) 1 μg kg–1 fentanil, grup A’ya (n = 33) 10 μg kg–1 alfentanil, grup R’ye (n = 36) 0,5 μg kg–1 remifentanil ve kontrol grubu C’ye (n = 39) salin, 2,5 mg kg–1 katkısız propofole ilave olarak 30 sn. üzerindeki sürede verildi. 90 sn. sonra LMA yerleştirildi. LMA yerleştirme kolaylığı değerlendirildi. Girişim sayısı, hava yolu sağlama kolaylığı ve hemodinamik değişiklikler kaydedildi.Article Comparison of the Effect of Bipolar Coagulation and Fibrillar Structured Oxidized Cellulose on Formation Epidural Fibrosis in Rats(Tubitak Scientific & Technological Research Council Turkey, 2021) CetIn, Eyup; Eren, Burak; Guzey, Feyza Karagoz; Tufan, Azmi; Tabs, Abdurrahim; ornek, Mustafa; cay, TugceBackground/aim: Epidural fibrosis (El) is a common cause of failed back surgery syndrome seen after spinal surgeries. The most frequent reason for the formation of BF is accumulated blood and its products in the operation zone. On the development of ER the effect of bipolar coagulation and fibrillar oxidized cellulose, which are used frequently to control bleeding, was investigated. Materials and methods: In the study, 45 male Sprague Dawley rats were divided into three groups (control, fibrillar, and bipolar). Lumbar laminectomy was applied to all rats under sterile conditions. In the control group, the epidural area was washed with saline solution. Bleeding was controlled with fibrillar oxidized cellulose in the fibrillar group, with bipolar coagulation in the bipolar group. The area to which laminectomy had been applied was removed as a block 6 weeks later and evaluated histopathologically and genetically in terms of EF development. Fibrosis degree was determined histopathologically by counting fibroblasts using the modified Lubina and EF He grading systems. Interleukin-6 (IL-6), transforming growth factor beta-1 (TG beta-1), and mRNA levels were measured by the droplet digital polymerase chain reaction method. Results: The number of epidural fibroblasts, percentage of modified Lubina, amount of 11-6, and He grading rates were significantly lower in the fibrillar group than in the bipolar and control groups (p < 0.05). On the other hand, there was no significant difference among the control, fibrillar, and bipolar groups in terms of TGF beta-1 values (p = 0.525). Conclusion: The use of fibrillar oxidized cellulose was more effective for hemostasis than bipolar coagulation in reducing the development of EE.Other Effect of Acute and Regular Exercise on Antioxidative Enzymes, Tissue Damage Markers and Membran Lipid Peroxidation of Erythrocytes in Sedentary Students(1998) Meral, İsmail; Bayıroğlu, Fahri; Şekeroğlu, M. Ramazan; Tarakcioglu, Mehmet; Aslan, Recep15 healthy sedentary men, 19-25 years old an did not have any programmed physical activity, were studied. The subjects were asked to run submaximal 15-20 min every day for 5 weeks. Erythrocyte lipid per-oxidation, Superoxide dismutase (SOD), glu-tathione peroxidase (GSH-Px) activities and glucose and uric acid levels were determined in blood samples which were taken before and immediately after acute exercise and after the end of 5-week training program. Malonaldialdehyde (MDA), sing of lipid perox-idation, creatin phosphokinase (CK), CK-MB, lactate dehydrigenase (LDH) concentrations increased (p<0.0001, p<0.01, p<0.05, p<0.05, respectively) and GSH-Px and SOD activities decreased significantly (p<0.0001 and p<0.05, respectively) after acute exer-cise. Although MDA level after the 5 week training program was lower than the MDA level after acute exercise period, it was still higher than sedentary period (p<0.01). GSH-Px activity after the 5-week training program was significantly higher than this of seden-tary period (p<0.0001). SOD activity after S-week training program was also higher than this of sedentary period but it was not statis-tically significant (p>0.05). We also found that uric acid and glucose levels increased immediately after acute exercise (p<0.05), but there was no significant differences between uric acid and glucose levels of seden-tary and training period. It is concluded that acute exercise causes oxidative stress in sedentary men. Thus irregular exercise, "weekend physical activities", may be harmful in contradiction with the common concept. On the other hand, regular exercise may pre-vent this deleterious condition by decreasing lipid peroxidation, augmenting antioxidant system and decrease the exercise-induced muscle damage.Other The Effect of Hepatic Plexus Vagectomy on the Sphincter of Oddi in Humans(1999) Karaayvaz, Muammer; Aydın, Metin; Dilek, Osman Nuri; Güler, OsmanRight upper quadrant billary type pain is classified in three types. Biliary group lll patients have only biliary type pain that is due to sphincter of Oddi dyskinesia and without any abnormalities. In these patients, endoscopic sphincterotomy is a current treatment method that is performed to decrease the choledochal pressure and to facilitate bile flow into the duodenum. In this prospective study, the effect of hepatic plexus vagectomy as an adjunct to cholecystectomy on the sphincter of Oddi was investigated in biliary group lll patients who were potential sufferers of postcholecystectomy syndrome. Common bile duct pressures in 62 cholelithiasis patients were recorded by means of a catheter introduced through the cystic duct after open cholecystectomy. The upper limit of normal choledochal pressure was hypothesized to be 15cm H 2 O. The perfusion pressures were above this limit in 15 patients. The first 7 patients formed the study group and the subsequent 8 patients were the controls. Hepatic plexus vagectomy was performed on the patients in the study group. The mean decrease of common bile duct pressure was 31.6% after hepatic plexus vagectomy and was significant (p<0.001). Neither right upper quadrant pain nor the pathologic symptoms due to vagectomy were reported in the study group during the follow up. Resisting right upper quadrant pain without an organic disorder was determined in 3 of the 8 patients in the control group and in 1 of the 47 remaining patients. Hepatic plexus vagectomy performed as an adjunct to cholecystectomy in biliary group lll patients eliminates the sphincter spasm that is the cause of biliary tract pain and it may be an alternative treatment to sphincterotomy.Article The Effect of Sildenafil on Recuperation From Sciatic Nerve Injury in Rats(Galenos Publ House, 2016) Korkmaz, Mehmet Fatih; Parlakpinar, Hakan; Ceylan, Mehmet Fethi; Ediz, Levent; Samdanci, Emine; Kekilli, Ersoy; Sagir, MustafaBackground: Severe functional and anatomical defects can be detected after the peripheral nerve injury. Pharmacological approaches are preferred rather than surgical treatment in the treatment of nerve injuries. Aims: The aim of this study is to perform histopathological, functional and bone densitometry examinations of the effects of sildenafil on nerve regeneration in a rat model of peripheral nerve crush injury. Study Design: Animal experiment. Methods: The study included a total of thirty adult Sprague-Dawley rats that were divided into three groups of ten rats each. In all rats, a crush injury was created by clamping the right sciatic nerve for one minute. One day before the procedure, rats in group 1 were started on a 28-day treatment consisting of a daily dose of 20 mg/kg body weight sildenafil citrate given orally via a nasogastric tube, while the rats in group 2 were started on an every-other-day dose of 10 mg/kg body weight sildenafil citrate. Rats from group 3 were not administered any drugs. Forty-two days after the nerve damage was created, functional and histopathological examination of both sciatic nerves and bone densitometric evaluation of the extremities were conducted. Results: During the rotarod test, rats from group 3 spent the least amount of time on the rod compared to the drug treatment groups at speeds of 20 rpm, 30 rpm and 40 rpm. In addition, the duration for which each animal could stay on the rod throughout the accelerod test significantly reduced in rats from group 3 compared to rats from groups 1 and 2 in the 4-min test. For the hot-plate latency time, there were no differences among the groups in either the basal level or after sciatic nerve injury. Moreover, there was no significant difference between the groups in terms of the static sciatic index ( SSI) on the 42nd day ( p=0.147). The amplitude was better evaluated in group 1 compared to the other two groups ( p<0.05). Under microscopic evaluation, we observed the greatest amount of nerve regeneration in group 1 and the lowest in group 3. However, this difference was not statistically significant. Moreover, there was no significant difference in the bone mineral density (BMD) levels among the groups. Conclusion: We believe that a daily single dose of sildenafil plays an important role in the treatment of sciatic nerve damage and bone healing and thus can be used as supportive clinical treatment.Article The Effects of Hyaluronic Acid on Myringoplasty in Rats(1999) Kutluhan, A.; Ugras, S.; Inalkac, E.; Akpolat, N.Tympanic membrane perforations arise from a variety of causes. Some of them require myringoplasty. In this study, we used hyaluronic acid, a polysaccharide with high-molecular-weight which may be obtained commercially and affects modulations of cell function. An experimental myringoplasty was performed on rats in which a chronic dry tympanic membrane perforation was formed. While physiological saline was embedded in Gelfoam® which was supported under and over the graft, in group 1, hyaluronic acid embedded gelfoam was used in group 2. Better epithelization, less calcification and scar tissue were established in group 2 myringoplasties. This study shows that hyaluronic acid may be used in myringoplasty especially in myringosclerotic tympanic membrane perforations.Other An Enzymatic Method for Isolating Cardiac Myocytes From Heart of Guinea-Pig Used for Single-Cell and Patch-Clamp Studies(1997) Meral, IsmailA method for isolating cardiac myocytes from the heart of guinea-pig is described. This method consists of perfusion of the heart with a low Ca{2+} medium and subsequent dispersion of cells with collagenase and proteolytic enzyme. More than 80% rod-shoped, viable and quiescent cells which are suitable for electrophysiological and pharmacological experiments are obtained by this method.Article Evaluation of Brainstem Auditory Evoked Potentials and Their Relationship With Levels of Thyroid Autoantibodies in Patients With Vitiligo(Tubitak Scientific & Technological Research Council Turkey, 2016) Cecen, Ilhan; Karadag, Ayse Serap; Tombul, Temel; Gunes Bilgili, Serap; Calka, Omer; Burakgazi, Ahmet ZubeyirBackground/aim: Vitiligo is a common depigmenting disorder. The damage can also occur in similar ways to melanocytes within other organs. We evaluated the brainstem and auditory pathway functions by evaluating brainstem auditory evoked potentials (BAEPs) and whether there is any relationship between auditory functions and autoimmunity. Materials and methods: Forty patients with vitiligo and 20 healthy volunteers were enrolled. Thyroid functions and autoantibodies were examined and BAEP tests were assessed by a neurologist. Results: Antithyroid peroxidase (anti-TPO) and antithyroglobulin (anti-TGA) antibody positivity was higher in the patient group (P < 0.05). A negative correlation was detected between anti-TPO and lead III, IV, and V latency and I-III interpeak latency (IPL) of the right ear together with lead IV latency and I-V IPL of the left ear in the patient group. When each BAEP parameter was compared between the two groups, more abnormalities were detected in the V latency and III-V IPL of the left ear together with IV and V latency of the right ear. Conclusion: In this study the presence of a correlation between increased anti-TGA and anti-TPO levels and BAEP parameters may be related to an autoimmune-mediated mechanism. However, further studies are needed to be performed in a large patient series.Article The Evaluation of Disease Awareness, Caregiver Burden, and Workday Loss in Caregivers of Copd Patients(Tubitak Scientific & Technological Research Council Turkey, 2022) Baha, Ayse; Kokturk, Nurdan; Ozturk, Burcu; Ozari Yildirim, Elif; Ozmen, Ipek; Gurgun, Alev; Ates, CanBackground/aim: Our aim is to determine the caregiver burden of chronic obstructive lung disease (COPD) patient's caregivers, and to determine whether there is a workday loss. Materials and methods: 252 COPD patients and their caregivers were included. Disease information of the patients were recorded and a questionnaire was applied. Socio-demographic characteristics of the caregivers were recorded and a questionnaire consisting of 24 questions including COPD disease, treatment and loss of working days, and the Zarit Scale were used. Results: 128(50.8%) of the patients according to GOLD were group-D, 97(38.5%) of the patient's relatives were working, 62(24.7%) were not able to go to work for 1-14 days, and 125(57.1%) spent outside the home from 1-14 nights, because those accompanied to patients. In univariate analysis were detected modified medical research council (mMRC) (p < 0.001), CAT (p < 0.001), the number of comorbidities of patients (p = 0.027), forced expiratory volume in 1 FEV1cc (p = 0.009), FEV1% (p < 0.001), the presence of long term oxygen therapy (LTOT), and the number of comorbidities of the patient's relatives (p = 0.06) increased the care load. In multiple linear regression analysis, age (p = 0.03), COPD assessment test (CAT) score (p = 0.001), FEV1% (<0.068) and the number of comorbidities of patients (p = 0.01) and the number of comorbidities of caregivers (p = 0.003) increased the caregiving burden. Conclusion: In COPD increases caregiving burden. This burden is greater in symptomatic patients and when comorbidities are present. Psychosocial and legal regulations should be investigated and solutions should be produced for the caregivers of COPD patients.Article Evaluation of Seroprevalence and Clinical and Laboratory Findings of Patients Admitted To Health Institutions in Gumushane With Suspicion of Crimean-Congo Hemorrhagic Fever(Tubitak Scientific & Technological Research Council Turkey, 2021) Gurbuz, Esra; EkIcI, Abdurrahman; Unlu, Ahmet Hakan; Yilmaz, HasanBackground/aim: To determine the seroprevalence and evaluate clinical findings and laboratory results of patients prediagnosed with Crimean-Congo hemorrhagic fever (CCHF) in Gumushane. Materials and methods: Included in the cross-sectional study were 362 patients (162 female, 200 male) between 0 and 94 years of age, who were followed up after receiving a preliminary diagnosis of CCHF between January 2011 and December 2019. Anamnesis, age, sex, clinical findings, laboratory results, epidemiological and clinical evaluations, severity criteria, risk factor reviews, and a comparison of the suspected negative cases with positive cases were analyzed retrospectively. Patients included in the study were evaluated as RNA positive by polymerase chain reaction (PCR) or IgM-positive by ELISA. Results: Of the 362 patients admitted to health institutions with a preliminary diagnosis of CCHF, 242 were diagnosed as CCHFpositive (66.9%). Moreover, 196 of those CCHF-positive patients (81%) were admitted to health institutions during the summer months. Statistical analyses revealed a significant relationship between the incidence of CCHF and patients who had been in contact with animals, lived in rural areas, and had engaged in farming and animal husbandry. In addition, fever, headache, diffuse bodily pain, nausea and vomiting, diarrhea, fever of 38 degrees C or higher, tachycardia, elevated ALT/AST, creatine kinase (CK), and lactate dehydrogenase (LDH) levels, leukopenia, and thrombocytopenia were detected in the CCHF-positive patients. Significant relations were found between this disease and these symptoms. However, there was no significant relationship between the statistical evaluation of the disease and bloody diarrhea, bodily bruises, rash, unconsciousness, gingival bleeding, hypotension, epistaxis, petechiae, splenomegaly, ecchymosis, hematuria, maculopapular rash, gastrointestinal system complaints, anemia, or elevation of the international normalized ratio and activated partial thromboplastin time duration, separately. Conclusion: Of the 362 patients, 66.9% (242) of those who received a preliminary diagnosis of CCHF were indeed CCHF-positive in Gumushane. It was concluded that CCHF remains an important endemic disease in Gumushane. In addition, elevated ALT/AST, CK, and LDH levels, leukopenia, and thrombocytopenia in patients presenting with headache, fever, fever of 38 degrees C or higher, generalized body pain, nausea/vomiting, diarrhea, and tachycardia will play a pivotal role in the preliminary diagnosis of CCHF.Article Gastrointestinal System Lesions in Children Due To the Ingestion of Alkali and Acid Corrosive Substances(Tubitak Scientific & Technological Research Council Turkey, 2015) Demiroren, Kaan; Kocamaz, Halil; Dogan, YasarBackground/aim: To examine esophageal and gastric lesions in children due to the ingestion of alkali and acid corrosive substances and to emphasize all related complications. Materials and methods: The reports of 103 children who ingested or were suspected to have ingested corrosive substances and who then underwent upper gastrointestinal endoscopic inspections were evaluated retrospectively. Results: Of the patients, the mean age was 41 +/- 3.6 months, and 57.3% were male. Vomiting was the most common symptom (44.7%). Eighteen different commercial products were defined as corrosive substances: 59.2% of them were alkali, 39.8% were acids, and 1% had a neutral pH. These corrosive agents most frequently contained sodium hydroxide, followed by hydrochloric acid, sodium hypochlorite, and sulfuric acid. Endoscopic inspections were abnormal in 68% of the cases. Esophageal lesions were observed in 56.3% of the patients, while gastric lesions were observed in 35%. During the follow-up period, esophageal strictures developed in 4.9% of patients, while gastric outlet obstructions developed in 1%. Engin; Mert, Guerkan; Petri, William A., Jr.Aim: The spectrum of clinical intestinal disease due to Entamoeba histolytica infection ranges from an asymptomatic carrier state to fulminant colitis with m array of manifestations. The extent of strain diversity for E. histolytica and the extent to which strains differ in virulence are not clearly known. in this study, we aimed to investigate the genetic diversity of E. histolytica isolates from Turkey. Materials and Methods: In order to study genetic diversity, the serine-rich gene of E. histolytica (SREHP) was investigated in 26 clinical isolates from Turkey by nested PCR amplification and restriction enzyme fragment length polymorphism (RFLP) analysis. Results: Twelve distinct DNA patterns were observed after Alul digestion of nested PCR products. The results demonstrate an extensive genetic variability among Turkish E. histolytica clinical isolates. Conclusions: Genotyping seems to be a useful epidemiologic tool to identify common-source outbreaks as well as strain-specific manifestation of an infection.Article Hematological Complications in 787 Cases of Acute Brucellosis in Eastern Turkey(Tubitak Scientific & Technological Research Council Turkey, 2008) Dilek, Imdat; Durmus, Ahmet; Karahocagil, M. Kasim; Akdeniz, Hayrettin; Karsen, Hasan; Baran, Ali Irfan; Evirgen, OemerAim: In this paper, we present the hematological findings of 787 cases diagnosed with acute brucellosis. Materials and Methods: Records of the cases seen between 1994 and 2006 were retrospectively investigated regarding hematological changes. Results: Three hundred sixty-five (46%) patients were male and 422 (54%) were female. Mean age was 32 (range: 11-78 years). Mean hematological values were as follows: Hb 12 g/dl (4-19), Htc 36% (12-56), leukocyte 6.9 x 10(9)/L. (0.5-25) and platelets 213 x 10(9)/L (9-617). According to differential counts, 34% and 14% of patients had mononuclear cell and neutrophil dominance, respectively. Fifty-six percent of patients were found to have anemia, alone or in combination with leukopenia or thrombocytopenia. Fourteen percent of patients had thrombocytopenia, 12% leukopenia, 5% pancytopenia, 4% leukopenia + thrombocytopenia, 0.5% acute hemolysis, and 0.1% disseminated intravascular coagulation. Restoration of thrombocytopenia and leukopenia and improvement in clinical situation were seen within one week and recovery of anemia occurred within 3-4 weeks. Conclusions: Hematological complications such as anemia and leukopenia are more frequently seen in acute brucellosis cases. In this report, we present a case with exudative pleural fluid first considered as tuberculous pleurisy due to lymphocyte predominance and elevated level of pleural adenosine deaminase, but later diagnosed as brucella pleurisy with positive serologic and microbiologic test results. Materials and methods: Biological activities of magnetic Au-nanoparticles have been observed to have the high biocompatibility, and a sample immunosensor model has been designed to use avidin attached Au-nanoparticles for L. monocytogenes detection. Staphylococcus aureus (S. aureus) and Salmonella typhimurium (S. typhimurium) bacteria cultures were chosen for control studies. Antimicrobial activity studies have been done to identify bio-compatibility and bio-characterization of the Au-nanoparticles in our previous study and capturing efficiencies to bacterial surfaces have been also investigated. Results: We constructed the calibration graphs in various population density of L. monocytogenes as 2.2 x 10(1) to 2.2 x 10(6) cfu/mL and the capture efficiency was found to be 75%. After the optimization procedures, population density of L. monocytogenes and Raman signal intensity showed a good linear correlation (12 2 - 0.991) between 10(2) to 10(6) cfu/ml, L. monocylogenes. The presented sandwich assay provides low detection limits and limit of quantification as 12 cfu/mL and 37 cfu/mL, respectively. We also compared the experimental results with reference plate-counting methods and the practical utility of the proposed assay is demonstrated using milk samples. Conclusion: It is focused on the enumeration of L. monocytogenes in milk samples and the comparision of results of milk analysis obtained by the proposed SERS method and by plate counting method stay in food agreement. This study aimed to analyze the tendencies of doctors who are not disease specialists (non-IDSs) towards the restriction of antibiotics. Materials and methods: A questionnaire form was prepared, which included a total of 22 questions about the impact of antibiotic restriction (AR) policy, the role of IDSs in the restriction, and the perception of this change in antibiotic consumption. The questionnaire was completed by each participating physician. Results: A total of 1906 specialists from 20 cities in Turkey participated in the study. Of those who participated, 1271 (67.5%) had <= 5 years of occupational experience (junior specialists = JSs) and 942 (49.4%) of them were physicians. Specialists having >5 years of occupational experience in their branch expressed that they followed the antibiotic guidelines more strictly than the JSs (P < 0.05) and 755 of physicians (88%) and 720 of surgeons (84.6%) thought that the AR policy was necessary and useful (P < 0.05). Conclusion: This study indicated that the AR policy was supported by most of the specialists. Ramazan; Tarakcioglu, Mehmet; Yılmaz, YükselWe aimed to reveal the value of serum CPK and LDH levels in he early diagnosis of testicular tortion in this study. 15 adult male New Zealand rabbits were divided into 2 groups. The first group included 7 animals that experienced sham operation and approved as control animals. In the second group (n=8), the left testes were just twisted 720 degrees and fixed by a transmesorchial suture like Ryan described. Blood samples were obtained by venapuncture at O, 4, 8, 24 hours and 4 weeks in each group; creatine phosphokinase (CPK) and lactate de-hydrogenase (LDH) were determined. There was slight increase in LDH level in the second group but not statistically significant (p>0.05). On the other hand, significant increase in CPK levels (p>0.05) in 4 hours after torsion were seen (mean 6297±2012 compared to the level in the first sample, 2768±982). CPK tended to decrease after 8 hours (mean 4459±1867). No significant alterations were noted in both enzyme levels in those having sham operation (p>0.05). Thus, determination of serum CPK may be an auxiliary alternative in the diagnosis of testicular torsion.
