The Frequency and Classification of Anemia in Patient Population Over 60 Years
Abstract
Anemi, yaşlılarda sık görülen, belirgin morbidite ve mortalitede artışa ve yaşam kalitesinde azalmaya neden olan önemli bir sağlık sorunudur. Toplumların giderek yaşlanıyor olmasının sonucu olarak gelecekte anemi prevalansının ve dolayısıyla sağlık sistemine yükünün hızla artması beklenmektedir. Bu çalışmada hastanemiz polikliniklerine başvuran 60 yaş ve üstü hastalarda anemi sıklığı ve nedenlerinin araştırılması amaçlandı.Çalışmaya Yüzüncüyıl Üniversitesi Tıp Fakültesi Hastanesi polikliniklerine başvuran 60 yaş ve üstü 1192 hasta alındı. Anemi tanısı, Dünya Sağlık Örgütü (DSÖ) kriterlerine göre serum hemoglobin değerinin erkeklerde 13 g/dL'nin, kadınlarda 12 g/dL'nin altında olması ile konuldu. MCV:77 fl ve altı mikrositer anemi, 78-95 fl arası normositer anemi, 96 fl ve üzeri makrositer anemi olarak sınıflandırıldı. Yaş ortalaması kadınlarda 69.7, erkeklerde 69.8 idi. Ortalama Hb değeri erkeklerde 13.7gr/dl, kadınlarda 13 gr/dl ölçüldü. Çalışmamızda 60 yaş üstü yaşlı populasyonunda anemi oranı % 28.5 olarak bulundu. Erkeklerde anemi kadınlardan daha sık gözlendi. Her iki cinsiyettede normositer anemi %72.1 oranında daha fazla saptandı. Onu sırasıyla mikrositer (%19.4) ve makrositer anemi (%8.5) izledi. Anemi en sık 80 yaş üstü hasta grubunda gözlendi. 60-70 yaş arasında erkeklerde anemi kadınlardan 1.89 kat daha sıktı. (p=0.001) Sonuç olarak Van ve bölgesindeki yaşlılarda anemi oranı diğer batı tabanlı yapılan çalışmalara göre daha yüksek bulundu. Etyolojiye yönelik daha geniş yeni çalışmaların yapılması gerekmektedir
Anemia, commonly seen in elderly people, is a major health problem which increases the morbidity and mortality and decreases quality of life significantly. As the society gets older the prevalence of anemia is expected to increase rapidly in the future and as a result the burden on the health system will increase correspondingly. This study aims to focus on reasons and frequency of anemia in 60 years and older patients applied to our hospital.In the scope of the study, 1192 patients at the age of 60 years and older were examined. The patients were chosen from people that applied to Yuzuncu Yil University Hospital. The patients were diagnosed according to criteria of the World Health Organization (WHO). When the serum hemoglobin value was less than 13 g / dL in men 12 g / dL in women, they were diagnosed as anemia. The anemia classification was done like that; MCV: 77 fl and below as microcytic anemia, between 78-95 fl as normocytic anemia, 96 fl and over as macrocytic anemia. The average age for women was 69.7 years and for men 69.8 years. The measured average Hb value was 13.7gr/dl in men and 13 g / dl in women. In our study the rate of anemia in population over 60 years was 28.5%. Anemia was observed more common in men than women. The denormositer anemia was higher with 72.1% rate in both sexes. The microcytic anemia with 19.4% and macrocytic anemia with 8.5% followed denormositer anemia. The anemia was most common in patient group over the age of 80. The anemia was 1, 89 times more common in men between the ages of 60-70. (p=0.001) As a result the rate of anemia in Van region is found higher than the results found in other western-based studies. Etiology oriented new studies with larger scale is advised.
