The Effect of Traction Treatment of Knee Osteoarthritis: a Prospective Randomized Controlled Trial

Abstract

Amaç: Bu çalışmanın amacı diz osteoartritli hastalarda traksiyon tedavisinin etkinliğini değerlendirmektir.Hastalar ve Yöntemler: Klinik ve radyolojik bulgulara göre diz osteoartriti tanısı konmuş olan 90 (73 bayan, 17 bay) hasta çalışmaya alındı. Hastalar üç gruba randomize edildi. Kontrol grubundaki 30 hasta, 15 seans hot pack ve kısa dalga diatermi ile tedavi edilirken, diğer 30 hasta aralıklı traksiyon, hot pack ve kısa dalga diatermi ve son 30 hasta da devamlı traksiyon, hot pack ve kısa dalga diatermi ile tedavi edildi. Bütün hastalar, (1) başlangıçta, (2) 15. uygulamadan hemen sonra ve (3) tedavi bitiminden 30 gün sonra olmak üzere, hasta grubundan habersiz değerlendirici tarafından üçer defa değerlendirildiler. Western Ontario and McMaster Universities osteoarthritis (WOMAC) indeksi, Lequesne fonksiyonel indeksi ve ağrı (VAS, 100 mm) ve goniyometre ile ölçülen dizin eklem hareket genişliği indeksleri değerlendirmelerde kullanıldı.Bulgular: Başlangıç değerleri ile karşılaştırıldığında, 15. uygulamadan hemen sonra WOMAC, Lequesne ve ağrı skorlarına göre her 3 grupta da anlamlı azalmalar gözlendi (p?0.05). Tedavi bitiminden 30 gün sonraki değerlendirmede ise kontrol grubuna kıyasla traksiyonlu gruplardaki düzelmeler (özellikle devamlı traksiyon ile) üstün bulundu (p?0.05).Sonuç: Traksiyon tedavisi, diz osteoartritli hastaların ağrı ve fonksiyonel durumlarını anlamlı şekilde iyileştirir. Traksiyon, gelecekte diz osteoartritinin mevcut tedavilerine ilave bir potansiyel terapötik ajan olarak önerilebilir.

Objective: The aim of this study was to evaluate the efficacy of traction therapy in patients with knee osteoarthritis.Patients and Methods: Ninety patients (73 female, 17 male) diagnosed as knee osteoarthritis based on clinical and radiologic findings were included in the study. Patients were randomized into three groups. Thirty patients were treated with intermittent traction, hot pack and short-wave diathermy and 30 patients were treated with continuous traction, hot pack and short-wave diathermy, while 30 patients in control group were treated with hot pack and short-wave diathermy for 15 sessions. All patients were evaluated three times by assessor who blinded to the intervention: (1) at the baseline, (2) immediately after the 15th application, and (3) 30 days after the end of the treatment. Western Ontario and McMaster Universities osteoarthritis index (WOMAC), Lequesne Funtional Index, pain intensity (as measured with 100 mm visual analog scale) and range of motion of knee (measured with goniometry) were the primary outcome measures of the study.Results: As compared to the baseline, significant decreases were observed in all three groups in terms of WOMAC, Lequesne Funtional Index and pain intensity scores, at the immediately after the 15th application (p?0.05). At the 30 days after the end of therapy evaluation, the improvements were found to be superior in the traction groups (especially by continuous traction) as compared to the controls (p?0.05).Conclusion: Traction treatment significantly improves the pain and functional status of patients with knee osteoarthritis. Traction therapy may offer a potential therapeutic adjunct to current knee osteoarthritis therapies in the future.
