Abstract

Hata Ağacı Analizi ve Sağlık Alanında Kullanımı. Van Yüzüncü Yıl Üniversitesi Sağlık Bilimleri Enstitüsü Biyoistatistik Anabilim Dalı, Yüksek Lisans Tezi, Van, 2022. Bu çalışmanın amacı, Hata Ağacı Analizi (Fault tree analysis, FTA) sürecinin genel özellikleri ile birlikte incelenmesi ve bunların nitel ve nicel birer tahmin aracı olarak kullanılabilirliğinin açıklanmasıdır. Hata Ağacı, (Fault tree) belirli bir sistemin durumunu ve çalışma süreci içerisinde meydana gelebilecek olası hatayı (arızayı, olumsuzluğu veya başarısızlığı) belirler. Olası hataları nedenleri ile birlikte, belirli bir gösterim kuralları çerçevesinde mantıksal olarak ilişkilendirir ve grafiksel olarak sunar. Hata ağacında genel olarak, ilgilenilen durum, tepe olay olarak adlandırılır ve bu tepe olay ile ilişkili olan süreç tanımlanır. Hata Ağacı oluşturma sürecinin birinci adımında sistem (sistemi oluşturan unsurlar, bunların işlevsel ilişkileri ve performans gereksinimleri) tanımlanır. İkinci adımda sonuç veya çıktı, analiz edilerek olayın tanımı ve sınırları belirlenir. Üçüncü adımda, sonuç (çıktı) olayının (arızanın, hatanın veya başarısızlığın) işlevsel düzeyinde, bir veya daha fazla nedene kadar izlenmesiyle hata ağacının oluşturulması tamamlanır. Dördüncü adımda, sonuç (çıktı) olayın olasılığı veya sıklığı hesaplanır ve son olarak sonuçlarının raporlanması yapılır. Çalışmada, COVID-19 hastalığı için bulaş riskini oluşturabilecek hataları belirlemek üzere uygulama yapılmıştır. Sonuç olarak, belirlenen problemlerde hata ağacının uygulama sürecine ilişkin önerilerde bulunulmuştur.

The aim of this study is to examine Fault tree analysis (FTA) process with its general features and to explain their usability as a qualitative and quantitative estimation tool. Fault tree determines the state of a particular system and the possible error (fault, negative or failure) that may occur during the working process. It logically relates possible errors with their causes within the framework of a certain display rules and presents them graphically. Generally, in the fault tree, the state of interest is called the top event and the process associated with this top event is defined. In the first step of fault tree construction process, the system (the elements that make up the system, their functional relationships and performance requirements) is defined. In the second step, the result or output is analyzed and the definition and limits of the event are determined. In the third step, the construction of the fault tree is completed by tracing the resulting (output) event (failure, fault, or failure) at the functional level to one or more causes. In the fourth step, the probability or frequency of the outcome (output) event is calculated and finally the results are reported. In the study, an application was performed to identify faults that may pose a risk of transmission for COVID-19 disease. As a result, suggestions were made regarding the application process of Fault tree in the identified problems.
