Performance Management and Its Applications in Health: the Case of Universty of Health Sciences Van Training and Resarch Hospital

Abstract

Sağlık sistemi içinde en çok harcamaların yapıldığı, önemli sağlık kurumları olan hastanelerin kendilerine tahsis edilen kaynakları verimli kullanması ve bu kaynakların maliyetlerini düşürerek etkinliğini arttırması önem arz etmektedir. Bunu sağlamak için sağlık harcamalarının büyük bir kısmını oluşturan hastane yataklarının ve insan gücünün etkili kullanımını sağlamak amacıyla sağlıkta performans uygulamasına geçilmiş ve sağlıkta performans yönetimi ve uygulamalarının önemi ortaya çıkmıştır. Bu çalışmada Sağlık Bilimleri Üniversitesi (SBÜ) Van Eğitim ve Araştırma Hastanesinin 2017, 2018, 2019 ve 2020 yıllarına ait performans değerlendirmesi yapılmıştır. Hastane performans değerlendirme kriteri olarak; hekim başına düşen günlük poliklinik sayısı, yatak başına düşen hekim oranı, yatak başına düşen hemşire oranı, hekim başına ağırlıklı ameliyat sayısı, yatak doluluk oranı, yatak devir hızı ve ortalama kalış gün sayısı araştırmaya dâhil edilmiştir. Hastane faaliyetlerini yerine getirirken mevcut performans yönetimi düzeyi ile Sağlık Bakanlığı kaynaklarına göre olması gereken performans yönetimi düzeyi karşılaştırılarak değerlendirme yapılmıştır. Hastanenin idari fonksiyonelliğini ve etkinliğini ortaya koymak ve performans kriterleri uygulanmaya çalışılırken tespit edilen eksikliklerin iyileştirilmesi için öneriler sıralamak böylece yönetimsel kararlarda faydalı olmak amaçlanmıştır. Çalışmanın evreni Türkiye'deki hastaneler olup, örneklem olarak Sağlık Bilimleri Üniversitesi (SBÜ) Van Eğitim ve Araştırma Hastanesi seçilmiştir. Belgesel Kaynak Tarama Yöntemi kullanılarak çalışma alanıyla ilgili geniş bir literatür taraması yapılmış böylece tezin teorik kısmı oluşturulmuştur. Çalışmanın özelinde yer alan SBÜ. Van Eğitim ve Araştırma Hastanesi'nin performans yönetimi ve uygulamaları konusu ise anılan hastaneye ait birincil kaynaklar (elektronik/fiziki rapor, belge, diğer resmi evraklar) baz alınarak yazılmıştır. Birincil kaynaklar değerlendirilirken, nitel araştırma yöntemlerinden olan 'Doküman Analizi' metodu yardımıyla tanımlayıcı istatistiksel modeller kullanılmıştır. Kapsamlı ve objektif bir sonuç elde etmek adına 2017-2020 yılları performans kriterlerinin çalışma sınırları içerisinde yer alması uygun görülmüştür. Ancak Sağlık Bakanlığı tarafından henüz 2020 yılına ait veriler yayınlanmadığından bu yıla ait veriler bilgi amaçlı olarak paylaşılmıştır. SBÜ. Van Eğitim ve Araştırma Hastanesi'nin performans kriterleri değerlendirilirken Sağlık Bakanlığı'na ait istatistik yıllıkları, verimlilik karne uygulaması gösterge kartları ve Sağlık Bakanlığı Taşra Teşkilatı Yataklı Tedavi Kurumlarına Yönelik Yatak ve Kadro Standartları Yönetmeliği esas alınmıştır. Yapılan değerlendirme neticesinde SBÜ. Van Eğitim ve Araştırma Hastanesi'nin hekim başına düşen günlük poliklinik sayısı ve cerrahi hekim başına düşen ağırlıklı ameliyat sayısının en fazla 2019 yılında en az 2020 yılında olduğu, tüm yıllarda uzman hekim ve hemşire ihtiyacının bulunduğu, Sağlık Bakanlığı istatistikleri ile karşılaştırma yapıldığında, yatak devir hızı ile ortalama kalış gün sayısının genel olarak verimli olduğu, yatak doluluk oranının ise standartların altında kaldığı, böylece kaynakların etkin kullanılmadığı ve yatakların atıl kapasite ile çalıştırıldığı sonucuna ulaşılmıştır.

It is important that hospitals, which are important health institutions where the most expenditures are made in the health system, use the resources allocated to them efficiently and increase the efficiency of these resources by reducing their costs. In order to achieve this, in order to ensure the effective use of hospital beds and manpower, which constitute a large part of health expenditures, performance in health has been implemented and the importance of performance management and practices in health has emerged. In this study, the performance evaluation of the University of Health Sciences Van Training and Research Hospital for the years 2017, 2018, 2019 and 2020 was made. As hospital performance evaluation criteria; The number of outpatient clinics per doctor per day, the rate of doctors per bed, the rate of nurses per bed, the number of weighted surgeries per doctor, bed occupancy rate, bed turnover rate and average number of days of stay were included in the study. Evaluation was made by comparing the current performance management level and the performance management level that should be according to the sources of the Ministry of Health while performing the hospital activities.It is aimed to reveal the administrative functionality and effectiveness of the hospital and to list suggestions for improving the deficiencies detected while trying to apply the performance criteria, so that it will be useful in managerial decisions. The universe of the study is hospitals in Turkey and University of Health Sciences Van Training and Research Hospital was chosen as the sample. A large literature review was made using the Documentary Source Search Method, thus the theoretical part of the thesis was formed. The subject of performance management and practices of the University of Health Sciences Van Training and Research Hospital, which is part of the study, was written on the basis of primary sources (electronic/physical report, document, other official documents) of the said hospital. When evaluating primary sources,Descriptive statistical models were used with the help of the 'Document Analysis' method, which is one of the qualitative research methods. In order to obtain a comprehensive and objective result, it was deemed appropriate to include the performance criteria for the years 2017-2020 within the study limits. However, since the data for 2020 has not yet been published by the Ministry of Health, this year's data has been shared for informational purposes. While evaluating the performance criteria of the University of Health Sciences Van Training and Research Hospital, statistical annuals of the Ministry of Health, efficiency report card application indicator cards and the Ministry of Health's Provincial Organization Bed and Staff Standards Regulation for Inpatient Treatment Institutions were taken as basis. As a result of the evaluation, the number of daily outpatient clinics per physician and the number of weighted surgeries per surgical physician in the University of Health Sciences Van Training and Research Hospital were at the most in 2019, at the least in 2020, and that there was a need for specialist physicians and nurses in all years, with the statistics of the Ministry of Health and that when a comparison is made, it has been concluded that the bed turnover rate and the average number of days of stay are generally efficient, the bed occupancy rate is below the standards, so the resources are not used effectively and the beds are operated with idle capacity.
