The Relationship of Pcna, C-Erbb2, Cd44s Expression With Tumor Grade and Stage in Urothelial Carcinomas of the Urinary Bladder

Abstract

Mesanede görülen tümörlerin büyük çoğunluğu ürotelyal karsinomlardır (ÜK). ÜK'lar erken yaşlarda da görülebilmesine rağmen genellikle ileri yaşlarda daha sık ortaya çıkarlar ve erkeklerde kadınlardan daha sık görülürler. Mesane tümörlerinde prognozu belirleyen en önemli faktörler histolojik derece ve evredir.Çalışmamızda, mesane ÜK'larında c-erbB2, CD44s ve PCNA immünreaktivitesinin histolojik derece ve evre arasındaki ilişkisi araştırıldı.Çalışmaya alınan örnekler Yüzüncü Yıl Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalına, 1999-2004 yılları arasında transüretral rezeksiyon (TUR) yapılarak gönderilmiş ve anabilim dalımızda ÜK tanısı almış 46 vakadan oluşuyordu. Vakalar histolojik olarak sınıflandırıldığında, dokuzu düşük malign potansiyelli papiller ürotelyal neoplazi (DMPPÜN), 23'ü düşük dereceli papiller ürotelyal karsinom (DDPÜK), 14'ü yüksek dereceli papiller ürotelyal karsinomdan (YDPÜK) oluşuyordu.İmmünhistokimyasal incelemede, derece ve evrenin artması ile c-erbB2 ve PCNA boyanma oranının arttığı izlendi ve bu iki markır ile tümörün derece ve evresi arasında istatistiksel olarak anlamlı ilişki gözlendi. Derece ve evrenin artması ile CD44s pozitifliği oranının istatistiksel olarak anlamlı bir şekilde azaldığı gözlendi.Anahtar Kelimeler:immünohistokimya

The Relationship of PCNA, c-erbB2 and CD44s Expression with Tumor Grade and Stage in Urothelial Carcinomas of the Urinary Bladder.The majority of the tumors seen in the urinary bladder are urothelial carcinomas. Eventhough some urothelial carcinomas are encountered in early ages, they usually appear in elderly and more often in men than women. The most important factors in determining the prognosis in the urinary bladder tumors are the histological grade and stages. In our study, the relation between the histological grade and stages and the immunoreactivity of the c-erbB2, CD44s and PCNA has been researched.The samples were obtained from the 46 cases undergone transurethral resection between the 1999-2004, which we diagnosed with urothelial carcinomas at Yuzuncu Yıl University School of Medicine Department of Pathology. The cases were classsified histolojically i.e. nine of these were papillary urothelial neoplasm of low malignant potential , 23 were low grade papillary urothelial carcinomas and finally the 14 of the cases were high grade papillary urothelial carcinomas.In the immunohistochemical examination, it was realized that the percentage of staning increased with the advancing of the grade and stage. There was a statistically significant relationship between the grade and stage of the tumor and these two markers. On the other hand, immunoreactivity of the CD44s decreased significantly with increasing grade and stage of the tumor.Key Words:Immunohistochemistry
