Evaluation of Proximal Femoral Nail Results in Patients With Femural Pertrocenteric Fracture Over 65 Age
Abstract
Bu çalışmanın amacı pertrokanterik femur kırıklarında PFN implantı uygulanan 65 yaş üstündeki hastaların klinik ve radyolojik sonuçlarını değerlendirmektir. Van Yüzüncü Yıl Üniversitesi Dursun Odabaş Tıp merkezi hastanesi Ortopedi ve Travmatoloji Anabilim Dalında Eylül 2009 ve Ekim 2019 tarihleri arasında 36 hasta incelendi. Çalışmaya alınan hastalar 20 erkek 16 kadından oluştu. Ortalama yaş 75,33(65-95)'idi. 18 hasta sağ 18 hasta sol taraflı idi. Çalışmamızda hastaların yaşı, cinsiyeti, kırık tarafı, yatış süresi, ameliyatın ilk yatıştan sonraki kaçıncı günde olduğu, cerrahi süresi, travma şekli, kırık tipi, ek kırık olup olmadığı, ASA skoru, anestezi tipi, cerrahinin açık veya kapalı uygulanma türü, harris kalça skoru, radyolojik kaynama süresi, komplikasyon, mortalite değerlendirildi. Çalışmada AO kırık tipi ile radyolojik kaynama süresi ve harris kalça skoru arasında anlamlı bir ilişki bulunmuştur. Kırık instabil ise sonuçların daha kötü olabileceği muhtemeldir. Prognozu belirlemede kırık tipinin önemini bir kez daha göstermiştir. Erkeklerde sol taraf, kadınlarda sağ taraf tutulumu daha yüksek oranda görülmüş; bu fark istatistiksel olarak anlamlı bulunmuştur. Bu konuda daha büyük olgu sayısı olan çalışmalara ihtiyaç vardır.
The aim of this study is to evaluate the clinical and radiological results of patients over 65 years of age who applied PFN implantation in pertrochanteric femur fractures. At Van Yüzüncü Yıl University Dursun Odabaş Medical Center Hospital, Department of Orthopedics and Traumatology 36 patients were studied between September 2009 and October 2019. The patients included in the study consisted of 20 men and 16 women. Average age was 75.33 (65-95). In our study18 patients were right, 18 patients were left sided. In our study; patients' age, gender, fracture side, hospitalization time, duration of surgery, type of trauma, type of fracture, whether there is additional fracture, ASA score, type of anesthesia, type of open or closed surgery, harris hip score radiological union time, complications, mortality were evaluated. In the study, a significant relationship was found between the type of AO fracture and radiological union time and harris hip score. If the fracture is unstable, the results are likely to be worse. It once again demonstrated the importance of fracture type in determining the prognosis. Left-sided involvement was higher in men and right-sided was higher in women; this difference was statistically significant. Studies with larger case numbers are needed on this subject.
