The Seroprevalence of Gluten Enteropathy Among Irritable Bowel Syndrome Patients

Abstract

Giriş: İrritabl barsak sendromu (İBS) toplumda sık karşılaşılan gastrointestinal sistem rahatsızlıklarındandır. Tanısı semptoma dayalı kriterlere (Roma I,II,III, Kruis skorlama sistemi,manning gibi) göre konulmaktadır. İrritabl barsak sendromu (İBS) ile gluten enteropatisi semptomlarında örtüşme olabilir.Amaç: Bu çalışmada, Roma III kriterlerine dayanarak tanı konulan İBS li hastalarda gluten enteropatisi seroprelevansını araştırmayı amaçladık.Hastalar ve yöntem: Çalışmaya İBS tanısı almış 188 hastanın (115 kadın, 73 erkek) dosyası retrospektif olarak incelendi. Çalışmaya dahil edilen tüm bireylerde Enzyme Linked Immunoabsorbant Assay (ELISA) yöntemi ile bakılmış olan doku transglutaminaz antikorları (tTGA) IgA (Immünglobulin A) ve IgG (Immünglobulin G) geriye dönük incelendi. Tüm hastalarda hemogram ve rutin biyokimyasal testler incelendi. IgA yetmezliği açısından tüm hastaların serum IgA düzeyi incelendi.Bulgular: Hastaların yaş ortalaması 39.93±17.118 yıldı. 188 hastanın 17'sinde (%9) tTG IgA antikoru pozitif saptandı. 6 hastada (%3.2) tTG IgG antikoru pozitif olarak bulundu. Hem tTG IgA hem de tTG IgG antikoru pozitif olan 2 olgu saptandı. tTG IgG antikoru pozitif, tTG IgA antikoru negatif olan 4 hastada serum total IgA seviyeleri normal bulundu.Sonuç: Gluten entertopatisi seroprevalansı İBS hastalarında normal populasyona göre daha yüksek bulunmaktadır. Objective: Irritable bowel syndrome is a frequent gastrointestinal disease in general population. Diagnosis is made with symptom based criterias (like Rome Criteria I, II, III, Kruis scoring system, Manning scoring system). There can be overlapping symptoms between irritable bowel syndrome and gluten enteropathy.Aim: In this study we aimed to investigate the seroprevalence of gluten enteropathy among irritable bowel syndrome patients, in which, the diagnosis was made according to Rome III criteria.Patients and method: In this study, files of 188 patients (115 female, 73 male) diagnosed as IBS, were reviewed retrospectively. Tissue transglutaminase antibodies ( tTGA) IgA ( Immunglobulin A), and IgG ( Immunglobulin G), which were gathered with Enzyme Linked Immunoabsorbant Assay ( ELİSA) method, were reviewed retrospectively among all patients in study population. Haemogram and routine biochemical tests, and for detection of IgA deficiency, serum IgA values were reviewed.Results: Mean age of patients was 39.93±17.11 years. Among 188 patients, tTG IgA antibody was positive in 17 of them ( 9%). In 6 patients tTG IgG was positive ( 3.2%). In 2 patients both tTG IgA and tTG IgG antibodies were positive. Serum total IgA values were normal in 4 patients, who were positive for tTG IgG and negative for tTG IgA antibodies.Conclusion: Seroprevalence of gluten enteropathy is more frequent among IBS patients in contrast to general population. As is the case in our study, it might be helpful to keep in mind the diagnosis of gluten enteropathy among IBS patients.Key words: Irritable bowel syndrome, gluten enteropathy, seroprevalence, Rome Criteria
