Article The 2011 Van Earthquake S and Resilience in the Urban Population(Istanbul Univ, Fac Letters, dept Geography, 2022) Choi, Jae hun; Alaeddiginoglu, FarukAbout a decade after the 2011 earthquakes, can people say that the city of Van has recovered now? This study purposes to answer this question by presenting a method for measuring the shock and process of recovering from an earthquake in terms of resilience and to provide objective results indicating whether or not Van has recovered from the shock of the 2011 earthquakes using this method. This study presents a method for measuring resilience in terms of population and local economic resilience. This study proposes the following five variables for measuring resilience in terms of population: decline year, drop (the degree of decline than expected), rebound (the velocity of recovery from decline), resilience index, and slope change (of the population regression equation). By synthesizing the resilience measurement results up to the end of 2019, the population growth rate of the whole city has yet to return to pre-earthquake levels. When measuring the resilience indicators at the neighborhood level, the neighborhoods that Housing Development Administration of Turkiye (TOKI) had built and the neighborhoods around the city center show high resilience. The resilience indicators for neighborhoods near TOKI were low. The conclusion is that appropriate measures have not been taken from the perspective of the entire city of Van to overcome the effects of the 2011 earthquakes.Article 3d Location Optimization of Multiple Unmanned Aerial Vehicle Base Stations in Next-Generation Cellular Networks and a New Meta-Heuristic Approach(Gazi Univ, Fac Engineering Architecture, 2021) Ozdag, RecepThe use of Unmanned Aerial Vehicles (UAVs) as mobile Base Stations (BSs) in wireless communication is emerging as an effective technique that should be used for the services planned in the next-generation cellular networks or in disaster situations. Currently, effective 3D placement of BS mounted on UAVs or Drones (Drone-BS), also called Low-altitude Air Platforms, in the defined area significantly increases the Quality of Service (QoS) of wireless communication. This study has been aimed to be performed dynamic deployments (location optimization) of Drone-BSs randomly distributed in the 3D plane in order to optimally cover the users in the urban environment according to the Air-to-Ground (ATG) model. Using new approaches based on the Electromagnetism-Like (EML) Algorithm and the Whale Optimization Algorithm (WOA), which are widely used in the literature and are meta-heuristic, it was planned to be covered the maximum number of users on the ground by multiple Drone-BSs. In addition, Extensive-interval Optimal Fitness Search Algorithm (EOFSA-EML) and Discrete-interval Optimal Fitness Search Algorithm (DOFSA-EML) approaches were developed based on the EML algorithm. Based on comparison metrics for multiple Drone-BSs distributed by EOFSA-EML, DOFSA-EML, and Pure-WOA, it has been found that EML-based algorithms achieve optimal results compared to Pure-WOA.Article 8-Ohdg Levels in Patients With Allergic Rhiniti(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2011) Onbasi, Kevser; Noyan, Tevfik; Cebi, Aysegl; Kiroglu, FarukObjective: Respiratory allergies like allergic rhinitis and allergic asthma are more frequently observed worldwide. Chronic inflammation in the respiratory tract is observed in these diseases. Oxidative stress contributes to this inflammation in the respiratory airways. 8-OHdG is a marker of oxidative stress. The aim of our study was to investigate the 8-OHdG levels in patients with respiratory allergy. Material and Methods: 23 patients with allergy consisting of 11 men and 12 women and 23 healthy controls were enrolled to the study. Results: 8-OHdG levels were found to be higher in allergic patients in comparison to healthy controls but it was not statistically significant. Conclusion: The reason for not significantly high levels of 8-OHdG levels as a marker of oxidative stress in respiratory allergy may attribute to the factor that the inflammation in respiratory allergy is less intensive in comparison to the inflammation in atopic dermatitis.Article Abstrackt Pictorial Images in the Teaching of Shaman(Anadolu Univ, 2011) Kiran, HasanImages could be regarded as internalized sensations. Artist can express shamanistic messages through myths in the form of symbols. Since shaman's world conserve its natural and unique mystery ever, artists tend to employ it in their works. Particularly shaman's unique, creative and natural dance, his inner trance state, ecstasic instrument playing, rich imagination, experiences and esoteric sayings arouse artists's interest. All these aspects could be utilized as essential elements in the world of imagery.Article Acute Cyanide Poisoning After Eating Apricot Pits: a Case Report(Galenos Yayincilik, 2012) Kaya, Avni; Okur, Mesut; Ustyol, Lokman; Temel, Hayrettin; Caksen, HuseyinThe stones and seeds of some plants such as apples, apricots, and peaches contain significant amounts of cyanide glycosides. Apricot pits are more toxic as they contain higher amounts of cyanogens and release hydrogen cyanide more easily. A previously healthy 27-month old male patient was admitted to our emergency department as intubated. His history revealed that he was intubated in the hospital where he was taken to with the complaint of fainting after having eaten numerous apricot pits with other family members. His general status was poor and he was unconscious. Both of his pupils were reactive. His deep tendon reflexes were increased and his plantar reflex was extensor bilaterally. The case was diagnosed as cyanide intoxication and the patient was admitted to the intensive care unit. Oxygen was constinuosly given under observation. After administration of hydroxocobalamine and sodium bicarbonate and correction of sodium deficit the patient regained consciousness and his general health improved. On the second day of admittance, he was discharged with a stable condition. This case was presented to emphasize that parents should not feed small children with apricot pits. (Turk Arch Ped 2012; 47: 141-2)Article Acute Respiratory Distress Syndrome Occurred After Hanging Result of the Attached To Rope: a Case Report(Aves, 2012) Kaya, Avni; Okur, Mesut; Akbayram, Sinan; Avcu, Serhat; Acikgoz, Mehmet; Dogan, Murat; Akil, MuhammedA-13-year-old male patient was admitted to our emergency unit because of near-hanging while playing with a rope. On physical examination, his general condition was not good, his state of consciousness was evaluated to be stuporous and his spontaneous respiration was weak. The patient had been intubated. The Glasgow coma score was 7. Rope marks were observed on his neck. Thorax computed tomography revealed widespread ground-glass appearance and bilateral parenchymal consolidation in the lungs. This findings correlated with acute respiratory distress syndrome. This case was reported for reminding that acute respiratory distress syndrome can be associated with near-hanging. (Turk Arch Ped 2012; 47: 302-4)Article Adjuvant Treatment Results and Prognostic Factors in Patients With Stage I-Iii Operated Breast Cancer(Kare Publ, 2006) Izmirli, Mustafa; Donmez Yilmaz, Binnur; Alan, Omur; Yalciner, Mehmet; Berberoglu, Elif; Unsal, MustafaOBJECTIVES We evaluated the adjuvant treatment results and prognostic factors in patients with breast cancer. METHODS A total of 197 stage I-III breast cancer patients (mean age 51; range 18 to 80 years) treated and followed up by Okmeydam Training Hospital, Department of Radiation Oncology in 1995 were evaluated retrospectively. RESULTS The 41.1% of patients were premenopausal and the remaining were postmenopausal. Invasive ductal carcinoma was encountered as the most frequent (85.8%) histopathologic type of tumor and most frequent (%49.7) stage was IIB. Modified radical mastectomy was performed on 65% of patients. In adjuvant treatment, cyclophosphamide-methotrexate-fluorouracil combinations and anthracycline containing combinations were given to 46.7% and 23.4% of patients respectively. In addition, endocrine therapy and radiotherapy was carried out on the 75.6% and 69.0% of patients. Local recurrences was occurred in a rate of 13.2% and distant metastases were observed in a rate of 26.4% and 7.1% of patients had both local recurrences and distant metastases. The 60.9% of patients were disease free and 1% with local recurrences also they were followed-up for a median time of 89 months (4-108). The overall survival rate and disease free survival rates for 5 and 8 years were as follows; 68%, 61%, 66%, and 60% respectively. With one variable analysis increased number of involved axillary lymph nodes, tumor size and stage gave rise to reduced overall survival rate and disease free survival rate. With multiple variable analysis, number of involved axillary lymph nodes (p=0.001) and tumor size (p=0.001) had significant effect on overall survival rate and disease free survival rate. CONCLUSION The tumor size and number of involved axillary lymph nodes were found to be statistically significant prognostic factors in breast cancer patients in our study.Article An Adult Form of Gaucher Disease Associated With Portal Hypertension: a Case(derman Medical Publ, 2013) Dulger, Ahmet Cumhur; Karadas, Sevdegl; Gonullu, Hayriye; Gultepe, Bilge; Tasdemir, MehmetGaucher disease (GD) is an inborn error of metabolism that affects the recycling of cellular glycolipids. Glucosylceramide (also called glucocerebroside) accumulate within the lysosomes of cells. Gaucher's disease is most common lysosomal storage disease and its incidence is 1/75.000. Three types of this disease have been defined. During the course of disease, it was reported that hepatosplenomegaly, portal hypertension, hyperferritinemia, splenic infarcts and splenic nodules might develop. Therefore, as in our case; Gaucher's disease must be remembered in the setting of hepatosplenomegaly, portal hypertension, hyperferritinemia, splenic infarcts and splenic nodules of unknown etiology.Article Algorithmic Approach To the Diagnosis and Management of Solitary Pulmonary Nodules(derman Medical Publ, 2015) Cobanoglu, UfukThe solitary pulmonary nodule is a common radiologic abnormality that is often detected incidentally. It is important to differentiate malignant nodules from benign nodules in the least invasive way and to make as specific and an accurate diagnosis as possible. In patients with a high probability of malignancy, surgery is the preferred strategy. Diagnostic algorithm for approaching the solitary pulmonary nodule, stratifying clinical risk factors in a standardized manner and blending this information with radiologic clues, would point the physician toward a benign or malignant cause. Such an approach would be expected to spare patients with benign causes the morbidity and cost associated with invasive tissue sampling and, at the same time, guide the physician toward recommending invasive tests for the nodules likely to be malignant.Article Amputations in Eastern Turkey (Van): a Multicenter Epidemiological Study(Turkish Assoc Orthopaedics Traumatology, 2008) Dogan, Ali; Sungur, Ibrahim; Bilgic, Serkan; Uslu, Mustafa; Atik, Bekir; Tan, Onder; Akpinar, FuatObjectives: The indications for amputations vary among countries and different regions depending on many factors. This study was designed to evaluate amputations performed in the province of Van and to determine specific causes of amputations associated with geographical and. cultural characteristics of the region. Methods: A total of 475 amputations were performed in 440 patients (345 males, 95 females; mean age 28.5 years; range 3 months to 85 years) in medical institutions of Van between 1995 and 2005. The patients were evaluated with respect to age and sex, etiology, side and level of amputations, and surgical interventions performed. Results: Amputations were the most common in the 21 to 30-year age group with 109 patients. Pediatric patients below the age of 10 years accounted for 13.4%. Of all the amputations, 218 (45.9%) involved the upper extremity, and 257 (54.1%) involved the lower extremity. Amputations were right-sided in 223, left-sided in 188 patients, and 29 patients underwent multiple amputations. Trauma was the most common cause of amputations (n=177, 40.2%), with leading etiologies as gunshot injuries (n=45), land mine (n=3.3) and hand grenade (n=7) blasts. Other frequent indications apart from traumatic causes were diabetes mellitus (n=86), congenital diseases (n=33), and peripheral vascular disease (n=30). Causes more specific to regional characteristics were tandoor burns (n=7), mistreatment by traditional bonesetters (n=3), and frostbites (n=3). Replantation was performed in 24 patients, of which 15 (62.5%) were successful. Conclusion: Our region features gunshot injuries and land mine blasts as the most common traumatic causes of amputations.Article The Analysis of Cardiologic Alterations in 70 Patients With Chest Trauma(Bilimsel Tip Publishing House, 2007) Cobanoglu, UfukWe here in present a prospective analysis of 70 patients with chest trauma (blunt or penetrating) admitted to the emergency service of Yuzuncu Yil University Hospitals from 9 March 2002 to 12 October 2003. The age range was 10 and 71 years, and 60 patients (% 86) were male. The control group consisted 16 healthy individuals (10 male, 6 female) aged from 15 to 69 years. Arterial blood gas analysis, pulmonary function tests, electrocardiography, echocardiography, blood hormone levels and blood biochemistry were evaluated for all patients at the time of their admission. Statistical comparison of the groups was performed. Physiological changes caused by chest trauma were evaluated.Article Analysis of Factors Affecting Dairy Farms' Animal Life Insurance: a Case Study Van Province(Kahramanmaras Sutcu Imam Univ Rektorlugu, 2024) Terin, Mustafa; Ceylan, Melike; Ciftci, Kenan; Yildirim, IbrahimGiven the unique characteristics of dairy cattle production, the producers are faced with many risks and uncertainties. These facts require efficient management for the survival of their farms. Agricultural insurance generally and animal insurance for dairy cattle farms specially is a significant tool to alleviate the losses and thus, contribute to the survival of the dairy farms. This study aims to determine the factors effective on the animal life insurance of the dairy cattle farms in Van Province. Face-to-face interviews with 178 producers through questionnaires were conducted and the data obtained was used for descriptive statistics, cross tables and binomial logit model. The logit model showed that having animal life insurance had positive effects on the annual milk production, keeping records in the farms, using agricultural credits, being a member of the Cattle Breeders' Association, and the number of support benefits while it had negative effects on the having tractor, cleaning the udder before milking, and having artificial insemination. We suggest that an increase in the participation of the animal life insurance could contribute to the sustainability and thus survival of dairy cattle farms in terms of increasing milk production and income of the producers in the province.Article Analysis of Factors Associated With Survival in Advanced Stage Gastric Adenocarcinoma(Kare Publ, 2005) Alici, Suleyman; Kaya, Serap; Izmirli, Mustafa; Tuncer, Ilyas; Dogan, Ekrem; Ozbek, Hanefi; Sayarlioglu, HayriyeIn this study, 138 patients with high grade gastric adenocarcinoma who were admitted to Yuzuncu Yil University, Medical School, Department of Medical Oncology between September 1999 and April 2002 were retrospectively evaluated in terms of the effects of clinicopathological parameters and treatment approaches on survival by single and multiple variable analyses. Patients histopathologically diagnosed as gastric adenocarcinomas with stage IV M0 (without distant organ metastasis) or stage IV M1 (with distant organ metastasis) were included in the study. Mean age of the patients was 61.1 (34-84), 64.6% male and 35.5% female. The ratio of patients with a performance score of 2-3 was 55%. The BMI was below 20 in 53.6% of the patients. According to histological grading, 41.9% of patients had indifferentiated adenocarcinoma, mucinous cell adenocarcinoma or signet ring cell adenocarcinoma. 84% of patients were clinically at stage IV M1. The percentage of patients with tumor localized upper 1/3 of stomach was 60.8%. While 19.6% of patients had received surgical treatment, 47% of the patients had received chemotherapy as adjuvant or paliative purposes. The percentage of the patients who responded to the therapy (complete response + partial response + stable response) was 39% and the percentage of patients who had progression was 7.2%. Overall median survival time was 3.1 months and three years survival rate was 8%. At single variable analysis, BMI, clinical stage, surgery, type of surgery and serum level of albumin were significant prognostic factors related to overall median survival time. Gender, clinical stage, surgery, type of surgery, hemoglobine concentration and serum level of albumin were found to be significant prognostic factors related to survival without progression (p<0.05). The factors found to be significant related to overall survival rate in single variable analysis were reevaluated in multiple variable analysis. No surgical treatment, paliative surgery (compared to radical surgery), BMI below 20 were found to be the statistically significant poor prognostic factors related to survival. The factors found to be significant related to survival without progression in single variable analysis were evaluated in multiple variable analysis and no surgical treatment (compared to surgery), male gender (compared to female) were found to be statistically significant poor prognostic parameters. In terms of both overall survival and survival without progression, whether performing surgery or not was statistically most significant independent prognostic factor. In conclusion, no surgical treatment, paliative surgery instead of radical surgery, and BMI below 20 on first admission were determined as poor prognostic factors related to survival in patients with high grade gastric adenocarcinoma.Article Analysis of Internet Addiction With Chaid Analysis: the Case of Van Province(Assoc Measurement & Evaluation Education & Psychology, 2011) Zirhlioglu, GurolThe purpose of a scientific study using existing data to be able to predictions about the unknowns and cause-effect relationship. The internet addiction level in Van and this dpendence effect factors to determine the configuration of this study the evaluation of internet addiction factors and those factors which at level of impact is high to determine the CHAID (Chi-Squared Automatic Interation Detection) analysis method was used.Article Analyze of Ventilator Associated Pneumonia(derman Medical Publ, 2015) Sunnetcioglu, Aysel; Karadas, Sevdegul; Cegin, M. Bilal; Sunnetcioglu, Mahmut; Kanter, AhmetAim: Ventilator-associated pneumonia (VAP) is the infection that is an important cause of morbidity and mortality developed in patients whom the invasive mechanical ventilation (MV) were performed in intensive care units (ICU). In this study, the factors of VAP developing in patients whom the mechanical ventilation of ICU performed, antibiotic susceptibility to these factors and determining the risk factors were aimed. Material and Method: Between January 2009 and March 2013, 79 cases, followed with the mechanical ventilation for at least for 48 hours and developed VAP, were retrospectively reviewed at Anesthesiology and Intensive Care Unit of Reanimation at Faculty of Medicine at Yuzuncu Yil University, performing endotracheal intubation. The cases were evaluated in terms of microorganisms, antibiotic susceptibility and risk factors. Results: The rate of our VAP speed was calculated to be 19.68 on the day of 1000 ventilator. While a single microorganism could be isolated in 81.1% of the 74 VAP cases whose the active pathogen could be isolated, two or more than two microorganisms were isolated in 18.9% of them. While 83 of the strains (90.2%) were gram-negative bacteria, 7 of them (7.6%) were gram-positive bacteria. Acinetobacter spp. (40.2%) was most commonly isolated as a gram-negative factor, but methicillin-resistant S. aureus (4.3%) was isolated as a gram-positive factor. It was determined that the isolated factors in VAP cases were significantly resistant to the broad-spectrum antibiotics. Discussion: As a result, in patients with high-risk factors for the development of VAP, early and appropriate empirical antibiotic treatment should be started according to the results of the sensitivity of the unit and for the multi-drug-resistant microorganisms with common and high mortality.Article Analyzing Factors Affecting Household Milk Products' Expenditures in Turkey: a Multivariate Heckman Sample Selection System Approach(Ankara Univ, Fac Agr, 2015) Terin, Mustafa; Bilgic, Abdulbaki; Guler, Irfan Okan; Yavuz, FahriIn this study, we analyze the effects of socio-demographic and economic factors of households on the consumption of milk and milk products by considering the recent increased economic trend in Turkey. The multivariate Heckman sample selection system (MSS) is used to analyze the data. The likelihood of spending and levels of expenditure on each foodstuff are jointly analyzed by assuming all pairwise cross correlation. The data used in this study are obtained from Turkish Statistical Institute (TURKSTAT) of 2010 Household Consumption Expenditures Survey (HCES). After discarding missing observations and outliers from the sample, the data finally contain 9442 individual observations. Results indicate that households with younger household heads (20-29 years) as compared households with household heads aged 60 and over are less likely to spend and spend less on milk, yoghurt, cheese with other dairy products in both regions. Families with children 0-5 years old are more likely to spend and spend significantly more on dairy products than that of families with other kids combinations in both regions. Families with annual income 15-30 thousand, 31-45 thousand and 45 thousand and over in Turkish Liras (TL) as compared to the base group (families with annual income 15 thousand TL) spend more 4.5 TL, 9.8 TL and 16.6 TL respectively on cheese and other dairy products in urban area, while these figures are 3.6 TL, 8.0 TL and 9.3 TL for rural households, respectively.Article Angiokeratoma of Fordyce in a Children(Turkish Soc dermatology venerology, 2014) Calka, Omer; Bilgili, Serap Gunes; Karadag, Ayse Serap; Bayram, Irfan; Ozdemir, SerhatAngiokeratomas are benign tumors characterized by epidermal hyperkeratosis, acanthosis and multiple dilated blood vessels in the papillary dermis. Angiokeratoma of Fordyce is one of five types in the group of the angiokeratomas, which occurs on the scrotum, penis or vulva. It is usually observed in young adults or elderly men. A 6-year-old boy presented to the dermatology department because of papular and erythematous lesions on his scrotum and penis. These lesions were found at birth and were asymptomatic. There was a history of occasional bleeding on trauma from the lesions. Histological evaluation of a skin biopsy specimen showed hyperkeratosis and acanthosis of the epidermis and multiple dilated thin-walled vessels in the papillary dermis. Based on the clinical, histopathological and dermoscopic findings, the patient was diagnosed with Fordyce angiokeratoma. Herein, we report a case of angiokeratomas of Fordyce, which is very rare in childhood and the dermoscopic findings.Article Animals and Our Animality From a New Aristotelian View(Beytulhikme Felsefe Cevresi, 2020) Mutlu, BarisWithin contemporary philosophy, animals have rightly been the subject of an important discussion. Singer started this process, and many important philosophers also participated in this discussion. One of these names, Richard Sorabji, put forward the following thesis: Western philosophy caused an important crisis especially in the field of moral philosophy and philosophy of mind with its approach towards animals. The emergence of this crisis in the field of moral philosophy is based on not knowing our moral responsibilities towards animals. Human beings put themselves at the top of the hierarchy of living things like value and consequently found the right to do what they wanted to the animals, and they could easily kill and slaughter animals for various reasons. Sorabji rightly begins this crisis with Aristotle, because he states that "humans" are separated from animals by "reason/rationality", which is a unique faculty for human beings, and that animals exist for humans by nature. Many philosophers have made the distinction between "human" and "non-human animal" a powerful philosophical tradition with such statements. However, revolts and objections against this distinction are increasing rightly. Environmental ethicists, Utilitarians, Kantians and Aristotelians have written works on this subject. In this study, we will focus on the Aristotelian philosophers Alasdair MacIntyre, Rosalind Hursthouse and Martha Nussbaum. With paying attention to the reading of these philosophers, we will emphasize that, even though Sorabji is based the crisis on Aristotle, the acceleration of the crisis is also about getting away from Aristotelianism, and that the first biologist, Aristotle, cannot be read in line with philosophers like Descartes.Article Animals and Our Certain Practical Obligations To Animals in Christine M. Korsgaard(Beytulhikme Felsefe Cevresi, 2019) Mutlu, BarisC. M. Korsgaard is a Kantian philosopher, she proceeds in her works with the guidance of Kant, and by this guidance, she argues important subjects as "agent and identity", "self", "normativity", "practical reason", and "animals". In her works she especially argues that normativity has a necessary relationship with human nature, it has a psychological power in human nature as a form of necessity, it functions as the creator of the human soul. Korsgaard believes that: Only the rational person reaches to "the normative principles, laws which will govern the things she/he does and believes", he/she has the skill "to normatively manage himself/herself". This skill is not available in other non-human animals; they all do not have a capacity of reason, rationality rather they only act with their evolutionary instincts, their natural structures. Korsgaard, however, says that we must not exaggerate this difference and that this difference does not make us morally superior to them. For animals, like us, value their lives, pursuit certain goods and ends, live with certain functions (ergon). In this paper, we will see the Kantian theory which Korsgaard argues about animals.Article Aortic Dissection in Case With Chronic Kidney Disease(Aves, 2017) Buyukaydin, Banu; Alay, Murat; Kazancioglu, Rumeyza; Ugur, Aycan; Tunc, Muhammed; Teker, Melike Elif; Erkoc, RehaAortic disection is an urgent clinical problem that rapid diagnosis and appropriate treatment is life-saving. Hypertension is one of the major risk factors. In this paper, a 56 years old female patient with hypertension and renal failure was presented. In this case after clinical suspicion aortic dissection was diagnosed with imaging studies. Endovascular aortic repair was performed but after surgery, patient died because of possible hemorrhage complication. In aortic disection, diagnosis and treatment could be fast. In chronic kidney disease the prognosis of these patients is adversely affected. Despite proper surgical intervention accompanying medical problems like chronic kidney disease have negative effects on survival.
